commercial services prepared in accordance with the format in the
Federal Acquisition Regulation (FAR) Subpart 12.6, as supplemented with
additional information included in the notice in accordance with 38
U.S.C 8153, Sharing of Health Care Resources with non-VA entities. This
announcement constitutes the only solicitation. Proposals are being
requested and written solicitations will not be issued. (2) This
solicitation is issued as RFQ 573-40-00. (3) The solicitation document
and incorporated provisions are those in effect through Federal
Acquisition Circular (FAC) 97-15. (4) This solicitation is advertised
as full and open competition with SIC: 8011 with a size standard of $
5 million. The procurement shall be conducted under FAR 13.5 Test
Program, simplified acquisition procedures, and a combination of FAR
12, 13, and 15. (5) [Line Item One]:The contractor shall furnish all
necessary supervision and qualified competent personnel to perform
physical therapy services at the Outpatient Center, Jacksonville,
Florida as administered by the Department of Veterans Affairs, North
Florida South Georgia Health System, VA Medical Center, Gainesville,
FL. The following FAR provisions apply to this solicitation: 52.212-1

Advertising. FAR clause 52.212-1 Instructions to Offerors -- Commercial
is hereby include. The following is an addendum to FAR clause 52.212-1;
Period for Acceptance of Offers, the amount of time an offeror must
hold its prices firm is hereby changed. An offeror must hold the prices
in its proposal firm for 60 calendar days from the date specified for
receipt of proposals. Offerors are advised that proposals may be
evaluated without discussions. Award will be based on price, past
performance and technical capability. Technical capability and past
performance, when combined, are more important than price. Offerors are
requested to submit documentation to support the technical capability
and past performance of their company and the individual that is
referred for placement to perform services. To be responsive to the
solicitation offerors shall submit credentials evidencing registration,
certification, and licensure, records of vaccinations and TB
immunizations of the individual(s) recommended for placement. The VA
intends to award a single contract. Options. The Government will
evaluate offers for award purposes by adding the total price for all
options to the total price for the basic requirement. The Government
may determine that an offer is unacceptable if the option prices are
significantly unbalanced. Evaluation of options shall not obligate the
Government to exercise the option(s). The Government intends to make
an award based on the best value to the Government by performing a
price/technical tradeoff analysis. In accordance with FAR subpart
9.104-1, each offeror is required to provide information on a
reasonable number of recent and relevant contracts for the same or
similar services, as well as the offerors primary financial
institution, to include contract numbers, verifiable points of contact,
telephone numbers and any other relevant information which will be used
by the Contracting Officer to make a responsibility determination about
a prospective offeror. FAR provision 52.213-3 Offeror Representation
and Certification -- Commercial Items, shall be completed and submitted
with the proposal. The contract base period will be for one year from
date of award, with four one year options for renewal. Total duration
of the contract will not exceed 5 years. The closing date for all
proposals is April 14, 2000 at 4:00pm local time. Offers in original
