a Combined Synopsis/Solicitation for commercial items prepared in
accordance with the format of FAR 12.6, as supplemented with additional
information included in this notice. This announcement constitutes the
ONLY solicitation. QUOTES ARE BEING REQUESTED AND A WRITTEN
SOLICITATION WILL NOT BE ISSUED. This Request for Quotation,
TOT-R-0331, incorporates provisions and clauses in effect through
Federal Acquisition Circular 97-15. The associated SIC is 8099 and the
business size standard is $5.M. The Office of Thrift Supervision (OTS)
is hereby soliciting quotes from offerors to establish an Elder Care
Program for OTS employees. The Contractor would: establish a dependent
care referral service that can help employees to locate caregivers, as
well as provide education materials on evaluating dependent care
alternatives for elderly relatives; have counselors available that can
provide employees specific answers about nursing homes or other
assisted-living communities, long-distance caregiving, agingand health
problems, legal issues, or financial concerns of the elderly and
understanding Medicare; assist in and help evaluate alternative living
arrangements or find respite in their own home and refer them to local
resources for further assistance; and give employees tips on how best
to cope with caring for elderly parents in their own home and refer
them to local resources for further assistance. The Contractor would
not recommend specific care programs, but act as a consultant to help
employees to determine what they need in this area and then help them
find it. Contractor would provide these services nationwide and have a
toll free number available for employees to call. Eligibility includes
all OTS employees (approximately 1300) and their immediate family
(spouse, parents, siblings and grandparents.) There will be No Waiting
Period. Utilization Rate is estimated to be about 200. The resultant
contract will have a base year and four one year option periods.
Interested offerors should provide as a part of their quote technical
material to reflect their experience in providing similar program in
the private/public sector; past performance information, including
clients names and telephone numbers; and a price for the program in
each year (Base, and Option Years 1 -- 4). Award Information: Award
will be on a best-value basis based on technical experience, past
performance and price. Offerors must also submit with the quote, in
accordance with FAR 52.212-3, a signed Offeror Representations and
Certifications -- Commercial Items. The certification can be found at
www.arnet.gov/far at Part 52. Additionally, the following FAR
provisions are applicable 52.212-1, Instructions to Offerors --
Commercial, 52.212-2, Evaluation -- Commercial Items, 52.212-4,
Contract Terms and Conditions -- Commercial Items, and 52.212-5,
Contract Terms and Conditions Required to Implement Statutes or
Executive Orders -- Commercial Items. Quotes are due at the following
