a change in the Request for Quotation Number and adds Note No. 1 for
100% small business set aside. B -- Evaluation Site Visits in Support
of All Trauma Data. The Consumer Product Safety Commission (CPSC)
intends to award three (3) Indefinite quantity, firm-fixed priced
simplified acquisitions to conduct on-site orientation/evaluation
visits for the National Electronic Injury Surveillance System (NEISS).
The NEISS program provides reports of consumer product-related
injuries/deaths from the statistically selected hospitals. The
Contractor(s) shall visit the selected site(s) and conduct an
orientation with hospital staff on the mission of CPSC and explain the
hospital's role in supporting the mission. The Contractor will be
required to perform an evaluation of the Hospital's NEISS data
collection efforts by conducting record reviews. The Contractor shall
conduct a record flow analysis, and identify problems with missing
records. The Contractor shall determine the completeness of the trauma
histories in the ER records, and shall perform a step-by-step review
of each item on Personal Computers and quality control functions. The
Contractor shall make arrangement to visit with the hospital Public
Relations staff. The Contractor shall perform a review of the
evaluation findings with the coder and supervisor. The Contractor shall
also perform an exit interview. Request for Quotation (RFQ) No.
CPSC-Q-00-1166 will be released on or about April 20, 2000 with
quotations due on or about May 5, 2000. All responsible sources may
request a copy of the RFQ by fax at (30l) 504-0628 and submit a written
quotation, which shall be considered by the agency. No telephone
requests for copies of the RFQ will be accepted. Competitive Simplified
Acquisition Procedures will be utilized in accordance with FAR Part 13.
The final award(s) decision will be based upon a combination of
demonstrated ability to perform the work, demonstrated ability to
provide high quality work products, quoted price, and past performance.
The anticipated award date is on or about June 5, 2000. The contract
period of performance will be approximately twelve (12) months. See
