Provide community-based residential care, treatment and rehabilitative
services for chronically mentally ill (CMI) veterans, with particular
focus on those who are homeless. Facility must be located within fifty
(50) miles of the Veterans Affairs Outpatient Clinic, Peoria, Illinois
(location of the VA Medical Center, Danville, Illinois' HCHV Program
& Coordinator). Contractor shall furnish the following basic
services: board and room; laundry facilities for residents; therapeutic
and rehabilitative services including structured group activities,
instruction in and assistance with health and personal hygiene,
monitoring of medications, supportive social services, individual
professional counseling, alcohol/drug abuse testing, and internalizing
knowledge of the illness/recovery process. Contractor shall arrange
for and/or provide a minimum of one urine screen per month per resident
and as many alcohol breathalyzer testing per resident as needed. The
contract facility must have a current occupancypermit or license as
required by the authority having jurisdiction. The prospective facility
or facilities must be inspected and approved by the VA HCHV inspection
team prior to contract award. Contract period shall be for one year,
with four one-year renewal options. Facilities interested in this
procurement must provide a statement in their request for bidding
documents as to exactly how they would be able to provide these
services, be within the fifty mile radius of the VA's HCHV Program, and
be able to begin services on or before June 1, 2000. Standard
Industrial Classification Code 8361 aplies to this procurement. A firm
is small if its average annual receipts for the preceding three years
do not exceed $5.0 million. Solicitation bidding documents will be
