Fortin @ 508-968-6521 GENERAL SCOPE OF WORK GENERAL This is a lead abatement project in the basement of Westchop Lighthouse Keepers Quarters 1 and 2, the purpose of which is to be fully qualified for a Letter of Full Deleading Compliance pursuant to State of Massachusetts 105 CMR 460.760(E). These are unoccupied portion of the units. All lead paint shall be removed per the State of Massachusetts requirements 454 CMR 22.00 and who shall be certified by the State of Massachusetts to perform such work. The Contractor shall provide all supervision, labor, tools, materials, transportation, equipment and other services as necessary to complete work in accordance with 454 CMR 22.00, this specification, attached drawings and applicable wage determinations. Work consists of, but is not limited to the following: a. State of Massachusetts 454 CMR 22.00 shall establish minimum work standards to protect the health and safety of all personnel involved; b. A written notification to all occupants, of work to be completed; c. Proof of proper certification and licensing of all employees in accordance with 454 CMR 22.00; d. Construction of barriers enclosing work site; e. Removal of all lead paint per 454 CMR 22.00; f. All lead paint and paint chips shall be transported legally by the contractor in DOT approved drums and turned over to USCG Air Station Cape Cod Environmental worker upon completion of work; g. Lead paint shall only be removed by State of Massachusetts lead abatement personnel; h. Lead abatement shall be performed as required by State of Massachusetts lead abatement 454 CMR 22.00; i. The contractor shall perform daily site cleanup; and a final site cleanup at the completion of work; j. Final site cleanup, testing by third party, transport of all lead contaminated waste to Air Station Cpae Cod, and acceptance inspection with the COTR present. The specified work will be done at West Chop Lighthouse 1 & 2, Martha's Vineyard, MA. A payment bond will be required for any awards over 25K prior to commencement. SIC code for this project is 1521. Contract commencement and completion date shall be established a the Pre-Construction meeting. Solicitation packages will be available form 4/4/00 to 5/10/00 at a cost of $23.30.
