jose.negron@med.va.gov. This is a combined synopsis/solicitation for
commercial service prepared in accordance with the format in FAR
Subpart 12.6 and 13.5 as supplemented, with the additional information
included in this notice. This announcement constitutes the only
solicitation; This is a Request for Quote and a written solicitation
will not be issued. Solicitation number RFQ 667-23-00; award will be
made using the simplified acquisition procedures. This solicitation
document and incorporated provisions and clauses are those in effect
through Federal Acquisition Circular 97-16. This acquisition is
unrestricted. The standard industrial classification (SIC) 7361.
REQUIREMENT: Contractor to provide one (1) Cardiologist, all inclusive
rate, to perform services, approximately 40 hours per week for
approximately six to nine months. Contractor shall provide all
liability insurance, services, lodging, transportation, communications
and associated expenses for the contract period. Individual performing
under this contract shall provide the necessary documentation for
privileging purposes at this VAMC. Services shall commence immediately
after award but not later than April 28, 2000. Cardiologist General
Diagnostic Services: (1) Administer, direct, perform and interpret
general diagnostic Cardiology to include routine diagnostic studies of
the heart and associated systems including Echo Cardiograms, thallium
studies etc; (2) Communicate pertinent patient data to charge
physician, as appropriate. (3) Recognize and initiate appropriate
measures in emergencies, as appropriate, including operation of
resuscitation equipment; (4) Possess knowledge of actions and usual
dosage of most commonly used emergency drugs and their usage; (5)
Assure content and correctness of prepared reports and sign documents
to validate their contents; (6) Provide professional direction and
teaching of Cardiology resident physicians in general diagnostic
Cardiology; and (7) Oversee the activities, teach, lecture, and direct
residents and technologists in specificareas of Cardiology.
QUALIFICATIONS: Personnel under this contract shall be licensed in a
State, Territory, or Commonwealth of the United States or the District
of Columbia and Board Certified. The qualifications of such personnel
shall also be subject to review by the VAMC Chief of Staff and
approval by the VAMC Director. Personnel assigned under this contract
shall be Board Certified or Board eligible. Board eligible candidates
must not exceed 4 years from the date they became eligible. Submit
copies of the individual credentials with your quote. TERM OF THE
CONTRACT: Services shall be provided at the Overton Brooks VAMC, 510
East Stoner Avenue, Shreveport, LA 71101, Monday through Friday from
7:30 am to 4:00 pm, except for federal holidays. National Holidays: The
ten (10) holidays observed by the Federal Government, i.e., New Years
Day, President's Day, Martin Luther King Jr.'s Birthday, Memorial Day,
Independence Day, Labor Day, Columbus Day, Veterans Day, Thanksgiving
Day, and Christmas Day. The contract resulting from this solicitation
shall be for a period of 6 to 9 months with the option to extend for
up to 6 additional one month periods. Additionally we have the option
to Terminate sooner in the event permanent recruitment efforts prove
successful. PAYMENTS: Sums due for actual work done, the Contractor
will be paid monthly, in arrears, upon receipt of a properly prepared
invoice FAR 52.232-25. The Contractor shall submit all invoices to the
following address: Chief Fiscal Service (04) Overton Brooks VA Medical
Center 510 East Stoner Ave. Shreveport, LA 71101. PERSONNEL POLICY: The
contractor shall be responsible for providing: -- Health examinations.
-- Worker's compensation. -- Professional liability insurance. --
Income tax withholdings. -- Social security payments. The parties agree
that the contractor shall not be considered a VAMC employee for any
purpose. CONTRACT PERFORMANCE: a. The contractor will be monitored
through a variety of mechanisms, including but not limited to: --
Clinic appointment lists showing patients seen. -- sign-in/out log. --
Chart documentation of patient diagnosis, treatment and or follow-up
procedure. -- QA monitors designed to assess the quality of care
provided in the clinic. b. The Chief of Medical Service (111) will be
responsible for monitoring the quality of services provided under this
contract. Estimated Hours 1024 _________ . Estimated Overtime 40
