The Health Resources and Services Administration intends to negotiate on a SOLE SOURCE basis with Medical Monofilament Manufacturing, LLC, 116 Longpond Road, Plymouth, MA 02360. This procurement shall be processed in accordance with the Simplified Acquisition Procedures in Part 13 of the Federal Acquisition Regulations. The purpose of this procurement is to deliver completed monofilaments to the Bureau of Primary Health Care (BPHC) for the administration of its Lower Extremity Amputation Prevention Program (LEAP). The contractor shall: submit artwork in response to the requirements for the BPHC LEAP monofilament; and deliver completed monofilaments to the BPHC. The Medical Monofilament Manufacturing, LLC is deemed the only responsible source capable of providing the required work and services. The period of performance is six months.
