This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. THIS ANNOUNCEMENT CONSTITUTES THE ONLY SOLICITATION; QUOTATIONS ARE BEING REQUESTED AND A WRITTEN SOLICITATION WILL NOT BE ISSUED. Responses shall reference "Request for Quotation No. DE-RQ04-00AL66655". This combined synopsis/solicitation document and incorporated provisions and clauses are those if effect through Federal Acquisition Circular 97-15. Although this requirement is anticipated to be awarded under full and open competition; the U.S. Department of Energy, Albuquerque Operations Office (DOE/AL) reserves the right to award on a 100% Small Business Set-Aside utilizing the Standard Industrial Classification (SIC) Code 5047 with a corresponding qualifying size standard of 100 employees (indicates maximum allowed for a concern, including its affiliates, to be considered small). Offerors responding should indicate whether or not they are a small business, small woman-owned business, small disadvantaged business, or 8(a). The DOE/AL intends to purchase the following (7) line items: Item 1 -- 8 Each, FirstSave AED or equivalent as detailed in the following specifications: five year lithium battery and manuals included with units; capable of defibrillating with Biphasic variable energy levels from 140 to 360 Joules which are optimized for patient impedance; single button operation for lay user simplicity; pre-connected electrodes to avoid manual dexterity task during high stress situations; comprehensive voice instructions to guide user through rescue operation; automated self checks at partial energy on a weekly basis and at full energy on at least a monthly basis; does not require daily or monthly user test or require routine annual service; and some type of "fuel gauge" for visual verification of battery power. FirstSave is manufactured by Survivalink. Item 2 -- 4 each, Software Kit including software, 1serial communication cable, and training video. Item 3 -- 8 each, Basic Life Support System including ready kit, oxygen kit, basic life support kit, and carrying bag. Item 4 -- 12 each, 2 year Electrodes. Item 5 -- 4 each, Training unit including training electrodes, training manual, and advance button. Item 6 -- 4 each, Training electrodes. Item 7 -- 4 each, Extended Battery.
