The provider shall provide for the delivery and management of primary medical care. Referrals for sub-specialty care and hospitalizations will be made at the Phoenix VA Medical Center. To be considered for award, the offeror must (1) have existed as a healthcare provider business entity providing similar services for a minimum of two years as of the date of submission of its proposal, (2) provide certification that all providers are licensed by the appropriate governing body, (3) have a minimum of multiple facilities located within the delineated area. Compensation will be made through monthly capitation payment. It is expected that services will begin within 30 days of contract award.
