commercial items prepared in accordance with the format in Federal
Acquisition Regulation (FAR) Subpart 12.6, as supplemented with
additional information included in this notice. This announcement
constitutes the only solicitation; proposals are being requested and a
written solicitation will not be issued. Solicitation No.
DTRS57-00-Q-80037 is issued as a Request for Quote (RFQ). This
solicitation is being conducted under Simplified Acquisition Procedures
(SAP), FAR Part 13.5. This solicitation document and incorporated
provisions are those in effect through Federal Acquisition Circular
97-16. The Standard Industrial Classification Code is 5047, and the
small business size standard is 100 Employees. The Volpe National
Transportation Syst ems Center (Volpe Center) has a requirement to
provide environmental engineering and remediation support to Region 8
of the Environmental Protection Agency (EPA). The Volpe Center is
supporting phase I environmental assessment tasks at the EPA s Libby
Asbestos Project. The Volpe Center is now preparing to assist EPA with
their planned biological risk assessment for residents of Libby,
Montana and the surrounding impacted area. This assessment is a logical
follow-up activity to the phase I assessment tasks conducted by the
Volpe Center. The purpose of this statement of work is to obtain
Government Furnished Equipment (GFE) for the planned biological risk
assessment. The requirement is for Radiographic System with the
following specifications: High Frequency Radiographic Generator wi th
the following minimum capabilities Generator rating (kW): to be
provided by the vendor; Anatomical programming; Self calibrating, self
diagnostic program; Automatic line compensation; Automatic shutoff;
Ratings (KV and mA stations) -- to be provided by the vendor; Automatic
Exposure Control; Tube heal protection. Radiographic Elevating Table
with the following minimum capabilities: Elevating (motor drive
hydraulic or Lead Screw); Four-way floating tabletop; Series 8000 Bucky
(or equivalent); Heavy duty cassette tray, size sensing; LPI and Grid
Ration to be provided by the vendor; Elevating range; Anti-collision
protection (table top); Table top dimension and lateral table top
travel capability -- to be provided by vendor; Table Load to be
provided by vendor. Floor Mounted Tube stand with the following
capabilities: Longitudinal, vertical and transverse motions; pivotal
and tube rotation; focal spot above floor; floor-mounted platform;
electromagnetic locks; Automatic collimator. Vertical Bucky with the
following capabilities: heavy duty, size sensing cassette tray; series
8000 bucky (or equivalent), right or left hand load; electromagnetic
locks; reciprocating grid, LPI and GRID Ratio to be provided by vendor,
medium focus, aluminum interspaced; LPI and GRID Ratio to be provided
by vendor. X-Ray Tube with the following capabilities: Focal spots,
Ratings and Dimensions to be provided by vendor; ~3.1 and ~14: compound
target; 200,000 heat unit anode; 90 degree arm angle. The quantity
required for this item is one. Delivery of this system will be 30 days
ARO to: St. John s Lutheran Hospital, 350 Louisiana Avenue, Libby,
Montana 59923. Proposals shall include the following information in a
written quotation: 1) a lump sum price for the system listed above. 2)
The brand name you are quoting on along with a complete list of
specifications; 3) Tax ID Number or Social Security Number; and 4) DUNS
No. The provision at 52.212-1, Instruction to Offers -- Commercial, is
hereby incorporated by reference. The provision at 52.212-2,
Evaluation -- Commercial Items: The Government will award a purchase
order resulting from this solicitation to the responsible offeror
conforming to the solicitation that will be the best value to the
Government, considering price and other factors. Offers will be
received on an all or none basis. A single award will result fro m this
solicitation. All Offerors must include a completed copy of the
provision at 52.212-3, Offeror Representations and Certifications --
Commercial Items, with their offer. The clause at 52.212-4, Contract
Terms and Conditions -- Commercial Items, is incorporated by reference.
The clause at 52.212-5, Contract Terms and Conditions Required To
Implement Statutes or Executive Orders -- Commercial Items, applies to
this acquisition; additional clauses cited in 52.212-5 that apply to
this acquisition are: 1, 6, 7, 8, 9, 11, 12, 13, 14, 16, 17, 22, 23.
