Blackburn, 414-902-5402 This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6 as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. The solicitation is being issued as a Request for Quote (RFQ). The solicitation number is RFQ #69D-137-00. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 97-14. The SIC code is 5047 and 8071 and is offered on an unrestricted basis. Great Lakes Acquisition Center, 5000 W. National Ave., Bldg. #5, Milwaukee, WI, 53295 is soliciting quotes and will award a firm fixed price contract on a competitive basis to the offerors whose quotation meets the requirements of this solicitation. The contract will be for one year from date of award with four one-year option periods. Requirements: Contractor will furnish all equipment, software (including upgrades), transportation, install/de-install, supplies (including reagents), training (for two VA operators), manuals and needed to process and interpret test results, and maintenance as follows: A) Provide a single analyzer that will provide all of the following assays at the estimated quantities on a cost per billable patient result: a) Erythropoietin [400/yr.], b) Intact PTH (Nichols) [1200/yr], c) Cortisol [1000/yr], d) A-TPO [800/yr], e) A-TG [800/yr], f) Calcitonin [50/yr], g) ACTH [84/yr], and h) IGF-1 [60/yr]. Note: a "billable result" includes all required reagents, controls (unless otherwise noted), consumables, and calibrators. The Contractor will install the analyzer in laboratory space on the 2nd floor of the Zablocki, VA Medical Center. The contractor must provide 24hr./day 7day/week technical support. The Contractor will invoice for services provided on a cost/billable patient result monthly in arrears. Invoices with be submitted to: Fiscal Service, Zablocki VA Medical Center, 5000 West National Ave., Building 111, Milwaukee, WI 53295. B) Vendors should supply the following information with their quotations: 1) complete description of the analyzer being proposed, 2) a list of at least three locations and contact points (including telephone number) where the proposed equipment is installed and working, 3) a brief resume of the firm providing the equipment and related services 4) a quotation for the amount of the services provided each year showing the pricing for each test on a price/billable patient result basis taking the annual estimated quantities in to consideration. Note: quantities shown are the Governments for the best estimates for this requirement. Award of the contract will be made to the vendor offering the best value to the government considering, the degree to which equipment and services meets the stated requirements, past performance, and price.
