synopsis/solicitation for commercial items prepared in accordance with
the format in FAR Subpart 12.6, as supplemented with additional
information included in this notice. The announcement constitutes the
only solicitation. The solicitation document and incorporated
provisions and clauses are those in effect through Federal Acquisition
Circular 97-16. Proposals are being requested and a written
solicitation will not be issued. The solicitation number is 00L350003.
The Beneficiary Medical Program (BMP), HHS/PSC through the General
Acquisitions Branch is requesting proposals to institute a full service
pharmacy managed care program for National Oceanic Services (NOS)
retirees and their dependants. The BMP provides advice and guidance to
NOS retirees and their dependants concerning: pre-authorization for
routine or non-routine health care services; issuance of purchase
orders to purchase health care services; management of their health
care entitlements; and access to health care services in both military
and civilian sectors. The BMP is looking to issue an award to a
contractor to institute a program to provide nationwide access via an
ID card for beneficiaries that will allow prescriptions to be filled at
network pharmacies. The program will ensure that the beneficiaries will
not have any out-of-pocket expenses for prescriptions or any claim
forms to return. The program must be able to deny certain prescriptions
based on an approved formulary. The contractor shall provide
consolidated itemized monthly invoices. The contractor shall be
compensated with a per prescription charge. NO CAPITATION FEE WILL BE
ACCEPTED. The period of performance shall be one (1) year with nine (9)
twelve (12) month options. In Fiscal year 1999, the beneficiaries
numbered 98 with 1,448 individual prescriptions. Individual
prescription charges totaled $86,994.51. Contractors shall submit a
capability statement documenting that the offeror has a commercial
program meeting the above criteria and a per prescription change. Award
will be made based on the lowest per prescription charge. The provision
at FAR 52.212-1 "Instructions to Offerors-Commercial Items" is hereby
incorporated by reference. The Provision 52.212-2
"Evaluation-Commercial Items" is not applicable to this solicitation.
Award shall be made based on the Lowest responsible per prescription
charge. Each Offeror shall include a completed copy of FAR 52.212-3
"Offeror representations and Certifications-Commercial Items" and their
DUNS Number. The FAR Clause 52.212-4 "Contract Terms and
Conditions-Commercial Items" and 52.212-5 "Commercial Items" are
applicable to this acquisition. Offerors can obtain copies of

small business set-aside, SICC Code 8099 $5.0 million. All proposals
