combined synopsis/solicitation for commercial items prepared in
accordance with the format in FAR Subpart 12.6 as supplemented with
additional information included in this notice. This procurement is
conducted under simplified acquisition procedures in accordance with
FAR Part 13. This announcement constitutes an official Request for
Quotes (RFQ) and a written solicitation WILL NOT be issued. The
solicitation document and incorporated provisions and clauses are those
in effect through Federal Acquisition 97-16. RFQ 658-35-00 should be
referenced on all quotes. The Standard Industrial Classification (SIC)
for this procurement is 7363 and the small business size-standard is
gross annual receipts NTE $5.0 million. This announcement constitutes
the only solicitation. The VA Medical Center, Salem, VA intends to
solicit quotes and award a firm fixed price contract on a competitive
basis under the authority of Title 38 USC Section 8153, to the
offeror(s) whose quotation meets the requirements of this solicitation.
The VAMC Salem reserves the right to award more than one firm fixed
price contract if it is determined to be most advantageous to the
government. The VAMC Salem, VA has an immediate need for services of a
commercial recruiting firm to conduct a contingency search as an
additional source of applicants for the purpose of referral of a Board
Certified/Board Eligible Physician(s) with special interest, training
and/or certification in Orthopedics, Urology, and Radiology.
Contractor shall provide professional recruitment services to recruit
one (1) board certified Urologist; (1) board certified Radiologist, and
(1) board certified Orthopedic physician for permanent staff physician
positions at the VA Medical Center, Salem, VA. Deliverables shall
include weekly progress reports to the Chief, Resource Support Service,
or his designee. Candidates must possess a current, full and
unrestricted license in a State, Territory, or Commonwealth of the
United States or the District of Columbia. Candidates must be Board
Certified or Board Eligible (candidates may be a Board Eligible
Physician if within four (4) years of postgraduate residency and/or
internship training) in Orthopedics, Urology, and Radiology, and must
meet the qualifications and credentialing/privileging requirements of
the VA Medical Center, Salem, VA. Candidates must be eligible to
practice as a bonafide physician within the United States or District
of Columbia. Candidates must be U.S. Citizen or permanent resident of
the United States. Firm(s) providing a quotation per this solicitation
are required to have sufficient experience in the permanent placement
of professional healthcare personnel. References shall be required
from the firm(s) upon award of any contract(s) that result from this
solicitation. Contractor shall be responsible for screening candidates
against the basic qualification requirements and verifying licensure
of all candidates before referring to Resources Support Service at the
VA Medical Center, Salem, VA. All candidates meeting these
requirements shall be referred. Offerors shall submit the following
with their quotation pricing schedule; curriculum vitae, medical
license (from a State, Territory, or Commonwealth of the United States
or the District of Columbia), evidence of board certification,
evidence of ACLS/BLS Certification and DEA certificate of proposed
physician(s). The salary (includes all specialty pay) ranges for the
required physicians hired within the VAMC, Salem system are: (1)
Orthopedic Surgeon is from $137,000.00 to $169,000.00, (2) Urology
Surgeon is from $131,200.00 to $169,000.00; and (3) Radiological
Physician is from $147,000.00 to $169,000.00, salary commensurate with
education and experience. Benefits include: thirty (30) days paid
vacation and fifteen (15) days sick leave annually, ten paid Federal
holidays, excellent retirement savings plan, and health & life
insurance shared expense. Malpractice insurance is not required for
employees of the Veterans Affairs. Selected physician(s) are required
to sign an agreement to work for the VAMC, Salem, VA for a period of
two years. If the selected physician(s) does not fulfill his/her
obligation to stay for the full two years, the physician shall be
responsible for repayment of all special pay received to the VAMC,
Salem, VA. The Contractor shall not issue any recruitment
advertisements identifiable to the Department of Veterans Affairs
and/or the VA Medical Center, Salem, VA without the approval of the
VAMC, Salem, VA. The VAMC, Salem, VA reserves the right to review,
reject, and/or modify any recruitment advertisement(s) proposed by the
Contractor prior to advertisement or publication. All advertisements
must contain a statement that the Medical Center is an EEO/MF
opportunity employer. Contractor may not imply that it is the sole or
primary avenue for employment with the Department of Veterans Affairs
or the VA Medical Center, Salem, VA. Review and approval of recruitment
advertisements may be accomplished by FAX to eliminate mail request.

FAX (540) 983-1049. The Contractor shall be responsible for requiring
candidates to complete VA Form 10-2850, Application for Physicians,
Dentists, Podiatrists and Optometrists. Application(s) shall be
forwarded to the Contracting Officer at the VA Medical Center, Salem,
VA once contractor has determined that candidate(s) meet all
requirements of the solicitation. Once recruited, applicants shall be
evaluated and considered for appointment by the VA through the
Department of Veterans Affairs accepted employment procedures. This
includes reference and background checks, credentials verification,
application of veterans preference, etc. Oral interviews shall be
conducted between the VAMC and the applicant prior to acceptance.
Candidate will be subject to successful completion of a pre-employment
physical examination and drug screening prior to employment.
Physicians employed by the VA as a result of this contract will work a
two-year probationary period. At the end of the two-year probationary
period, the physician will be reviewed by the VA Physician
Professional Standards Board. If acceptable level of performance has
been demonstrated, the physician will become a permanent employee of
the VAMC, Salem, VA. The VAMC, Salem, VA is actively recruiting for all
three positions listed in this synopsis/solicitation through normal VA
procedures. The Contractor shall receive no fees for applicants who
apply directly to the VA Medical Center, Salem, VA without having been
referred by the Contractor. Applicants may apply directly to the VA
Medical Center, Salem, VA and thus need not apply through the
Contractor. Candidates who have applied directly will be given the same
consideration as those referred from the Contractor. The VAMC, Salem,
VA shall not be responsible for any type fees due the Contractor by the
applicant. The Contractor shall refer only those applicants from whom
the firm or service has not accepted fees other than registration type
fees that are imposed regardless of whether the registrant is referred
for employment or placed. Upon award of contract, the VA shall furnish
to the contractor VA Form 10-2850s, Application for Physicians,
Dentists, Podiatrists, and Optometrists, Fact Sheet on the VA Medical
Center, Salem, VA, and other pertinent information regarding employment
with the VAMC, Salem, VA. PAYMENT: Contractor may provide multiple
applicants, however, payment will only be made if a successful
applicant is chosen from those offered. Recruitment services are for
three staff physician positions (Orthopedist, Urologist, and
Radiologist). The VA shall pay the Contractor one half ( ) of the
agreed fee upon acceptance of the Physician(s) by the Chief of Staff of
the VA Medical Center, Salem, VA and upon signing by the physician of
the commitment letter of employment. The remaining fee balance shall be
paid at the completion of the Physician(s)'s ninety -(90) calendar day
performance evaluation(s). Pricing Schedule: (1) Commercial recruiting
service for the position of Orthopedist $__________________ cost per
position. (2) Commercial recruiting service for position of Urologist
$_______________cost per position. (3) Commercial recruiting service
for the position of Radiologist $__________________ cost per position.
Total Cost for Recruiting Services $____________________. (Offeror do
have to submit a quote for all three positions to be considered
responsive). The following provisions apply to this solicitation:
52.212-1, Instructions to Offerors-Commercial, 52.212-4, Contract Terms
and Conditions-Commercial, 852.270-4 Commercial Advertising, 52.212-5,
Contract Terms and Conditions required to Implement Statutes or
Executive Orders-Commercial, including clauses incorporated by
reference under Section b (1)(8)(11-16)(23-24). The government intends
to evaluate offers and award without discussions. However, the
government reserves the right to conduct discussions later if
determined necessary by the Contracting Officer. The information
required in provisions 52.212-1, a completed copy of provisions
52.212-3, Offeror Representations and Certifications-Commercial Items,
and 852.219-70, Veteran Owned Small Business certification must be
submitted with offer. Offerors may find the FAR provisions and clauses
on the Internet at www.arnet.gov/far/. Quotes are due by close of
business, 4:30 p.m. local time, Friday, May 5, 2000. The contract is
for a six- (6) month period but may be extended for an additional six
months. Proposals shall be submitted to the address listed herein or
faxed to (540) 983-1065, and directed to the attention of Contracting
Officer (90C1). The Government is not responsible for the legibility of
any faxed documents. This procurement is unrestricted. Solicitation
