The Department of Housing and Urban Development (HUD) plans to issue a Request for Quotations under the Simplified Acquisition procedures for a contractor interested in performing forms design work for the Department. The objectives of the Purchase Order will be to provide form design services for the department so that both HUD employees and HUD clients will be able to complete forms and save electronic files for any form, with data, at the lowest cost. The Department needs contractor assistance for design and revision of its inventory of forms and for development of screen-fillable forms for use on the HUD web page. The principal application currently used by HUD for this purpose is Adobe PagerMarker 6.5 (PM65) and related Adobe software. HUd has an inventory of approximately 2300 forms of which about 1200 are currently in a PM65 electronic file; about 500 may under go some type of revision during the year. There are about 225 screen fillable files currently posted on the HUD web page and approximately 24 new forms are created yearly. The contractor also may be requested, on an as needed basis, to provide forms design in other applications such as Microsoft Word and Excel. These additional design services shall be performed upon the issuance of a work request. A Formal Request for Quotations will be issued on or about May 14, 2000. All request for the RFQ shall be faxed, on company letter head, to Catherine B. Pinkney, (202) 708-2933. NO TELEPHONE OR E-MAIL REQUEST WILL BE ACCEPTED. HUD posts all required notices of its contracting opportunities on its Internet Home Page Contracting Opportunities site at http://www.hud.gov/cts/ctsoprty.html. Potential offerors who download copies of solicitations from HUD's Home page will NOT automatically receive any amendments to those solicitations.
