Control and Prevention (CDC) intends to purchase a Roche/Hitachi 902
Chemistry Analyzer. CDC believes that its requirements are met by only
one provider, however all responsible sources that can meet the
requirements listed herein may submit a quote which will be considered
by the agency. Description of the requirements are: 1.) must have a
menu of 90 manufacturer supported methods, featuring two-part reagent
configurations. All reagents packaged, sold, supported and invoiced by
instrument manufacturer; 2.) must run the entire menu that is offered
on Hitachi System except B12 and folate; 3.) must be able to
accommodate five different sample types (serum/plasma, urine, CSF,
Supernatant and "other"); 4.) must have the ability to accommodate both
low and high range patient samples; 5.) must be reliable and have 2PM's
done per year by manufacturer; 6.) computer must be bi-directional; 7.)
must possess a small foot print of only 28 inches square and 34 inches
tall; 8.) User friendlysoftware, which is trained on-site; 9.) system
must provide 21 methods on board including electrolytes; 10.) must
provide the ability to quantitate the amount of bilirubin, lipemia and
hemoglobin in patient samples automatically and flag interference
levels which can adversely effect results; 11.) 98% uptime guaranteed;
12.) fully automated, random access, open chemistry, discrete analyzer
with refrigerated reagent compartment; 13.) use same ISI cartridges as
the Roche 912 chemistry analyzer; 14.) sample bar code reader onboard
analyzer; 15.) 48 on-line reusable cuvettess. Delivery must be made
within 30 days of purchase. THIS IS A COMBINED SYNOPSIS/SOLICITATION
FOR COMMERCIAL ITEMS PREPARED IN ACCORDANCE WITH THE FORMAT IN SUBART
12.6, AS SUPPLEMENTED WITH ADDITIONAL INFORMATION INCLUDED IN THIS
NOTICE. THIS ANNOUNCEMENT CONSTITUTES THE ONLY SOLITICTATION; PROPOSALS
ARE BEING REQUESTED AND A WRITTEN SOLICITATION WILL NOT BE ISSUED. The

Instructions to Offerors -- Commercial Items applies to this
acquisition; 52.212-2, Evaluation -- Commercial Items; the evaluation
criteria stated in paragraph (a) of this provision are as follows: (1)
technical capability of the proposed system to meet the requirements
stated herein, and (2) price. The clause 52.212-4, Contract Terms &
Conditions -- Commercial Items applies to this acquisition. The
following FAR Clauses are hereby added to this clause: 52.211-16 (0%
increase, 0% decrease, all items); 52.247-35 F.O.B. Destination; The
clause at 52.212-5 -- Contract Terms and Conditions Required to
Implement Statutes or Executive Orders -- Commercial Items applies to
this acquisition. The following clauses listed in paragraph (b) of
52.212-5 apply to this acquisition: (7), (9), (10), (11), (14), (16),
