This is a combined solicitation/synopsis for commercial items
prepared in accordance with the format in FAR Subpart 12.6, as
supplemented with additional information included in this notice. This
announcement constitutes the only solicitation; Request for Quotations
are being requested and a written solicitation will not be issued.
Solicitation number F7389400690200 is a Request for Quotation. This
solicitation document and incorporated provisions are those in effect
through Federal Acquisition Circular 97-15. This solicitation is
unrestricted. Respondents must currently perform this type of service
and have the required clearances. All work shall be performed in
accordance with the following statement of work. STATEMENT OF WORK:
CHARLESTON AIR FORCE BASE SOUTH CAROLINA YOUTH PROGRAMS SUMMER CAMP 1.0
SCOPE: Non-personal services to staff and operate Summer Day Camp
Program at Charleston AFB, SC. Camp Staff will augment current Youth
Program staff. 2.0 SPECIFICATIONS: STAFFING FOR CAMP PROGRAM: A.
Staffing for Camp Program will consist of junior, and/or graduate level
students majoring in recreation/leisure services, physical education
and/or other persons with knowledge and skills to add professionalism
to the program. Positions at potential programs are: 1-Camp Director ;
7-Camp Counselors. (1) Academics for the position of Camp Director at
a minimum are: Be a degree recipient in recreation and/or leisure
services curriculum from an accredited college or university; or
possess appropriate professional experience and combination of college
education in recreation and/or leisure services or related area;
professional experience in camp environments or two years practical
work experience in the recreation or related field of appropriate
degree. (2) Academic requirements for the position of Camp Counselor as
a minimum are: Junior/senior standing at the college/university
majoring in the appropriate field of study; i.e.,
recreation/leisure/physical education, or related field; must have
completed undergraduate studies in the area of assigned
responsibilities, i.e., sports, arts and crafts, or social programming;
have previous experience as youth counselor in summer programming or
experience related to youth counseling at state/federal level. B.
Provide one (1) Project Director who shall review and improve overall
programs and activities and recruit/select the camp staff. Academic
experience requirements for the position of Project Director are as
follows: Recipient of a Doctorate Degree in recreation/leisure and/or
parks and recreation or a Master_s Degree in recreation and/or leisure
services; experience as Youth Camp Director; two years experience in
developing programs in recreation/leisure services field. The Youth
Activities Director/Assistant Director/School Age Coordinator will be
responsible for on site supervision and operation of camp. 3.0 SCHEDULE
OF WORK: Camp is scheduled to operate from 12 Jun _ 11 Aug 00, Monday
through Friday, 0800 to 1700 daily. 4.0 SUPERVISORY CONTROLS: Counselor
to day camper ratio is one staff to 12 campers. Youth to be served in
the camps are U.S. Military and eligible civilian dependent youth 5-12
years of age. Estimated number of campers is 80. 5.0 CAMP PROGRAMS: It
shall be the responsibility of the School Age Coordinator, Camp
Director and Counselors to plan, organize, schedule and conduct a
well-rounded summer program that offers children/or adolescents an
opportunity to engage in a variety of age appropriate recreational
activities that foster the mental, social and physical well-being of
each child and assists youth in overcoming the stress of relocation and
adjustment to new peer groups. The program will offer opportunities for
creative and expressive experiences, provide for the development of
independence and feeling of self-worth, and reflect geographic, ethnic
and cultural differences. The program shall include holiday
programming where applicable. (i.e., 4th of July). A. Social
Activities: Skits, refreshments, demonstrations, lessons, table games,
reading time, cooking, nature walks, group participation activities,
individual growth and self-awareness programs, and participation
awards. B. Leisure Arts and Crafts: Sculpture, creative painting,
drawing, printing nature crafts, mosaics, woodworking, fiber art, and
other activities that foster a positive learning experience. C. Sports:
Activities are focused on helping children and youth learn sport
skills, develop fitness, sportsmanship and self-confidence. 6.0 QUALITY
ASSURANCE: A. Unless otherwise specified in the contract, the
contractor is responsible for the performance of quality assurance
functions to ensure programs and operations meet the contract
requirements. The government reserves the right to observe and/or
monitor the contractor_s work performance to assure those services
conform to prescribed requirements. B. The Project Coordinator shall
observe and evaluate the entire program at least once during the
program session and provide an oral and written report on findings and
recommendations to the appointed installation designate (i.e., Youth
Director, Assistant Director, and School Age Coordinator). C. All
contract staff must meet standard youth activity guidelines in
reference to health requirements such as free of communicable disease,
drug abuse, and child abuse. (1) Staff must comply with requirement
for a review of personnel and security records to include an FBI
fingerprint check and State Criminal History Repository Check (SCHR)
checks of residences listed on employment and security application. At
a minimum SCHR check, communication in writing will be made with each
state, providing full identifying information concerning each
applicant and requesting confirmation that the individual has not been
convicted in that state of a sex crime, an offense involving a child
victim, substance abuse felony, or a violent crime. Contractor
employees will submit a completed Standard Form 85P, "Questionnaire for
Public Trust Positions", which will be provided to the Contractor by
the NAFI as Government Furnished Property. The reason for request will
be identified as OTHER and annotated as CHILD CARE. The FD Form 258,
"FBI Applicant Fingerprint Card" and the completed Standard Form 85P
shall be forwarded for processing to: Defense Investigative Service,
Personnel Investigative Center, Attn: Air Force Liaison, 881 Elkridge
Landing Road, Linthecum MD 21090. (2) If an SCHR check is not returned
by the state(s) within 60 days, a final determination may be made
based on the FBI fingerprint check and available background
information. While background checks are processing, staff may be
provisionally hired as long as they are within the line-of-sight of an
individual with a satisfactorily completed background check. Contract
employees have the right to obtain a copy of the background check
report and to challenge the accuracy and completeness of the
information contained in the report. Contractor employees who have
previously received a background check must provide proof of the check
or obtain a new one. (3) Staff have had a physical examination within
the last 3 years and have the required immunizations for the area
where they will be assigned. Personnel assigned to OCONUS locations
will be screened for medical/emotional, or special needs related
conditions that may require special medical support or modified
housing. D. All counselor interns shall be trained to administer basic
first aid and cardiopulmonary resuscitation (CPR) and be certified as
qualified by a competent authority such as Red Cross. Counselors must
be trained in Red Cross Basic Lifeguarding and/or Emergency Water
Safety. This training will be provided to the counselor interns by the
Contractor, at no cost to the NAFI, prior to the counselor interns
working in the field. Desired arrival date is 5 June 2000. Camp start
date is 12 June 2000; camp end date is 11 August 2000. The provision at
FAR 52.212-1, "Instructions to Offerors -- Commercial Items" is
applicable to this solicitation. The provision at FAR 52.212-2 "
Evaluation -- Commercial Items" is applicableto the solicitation and
the blanks in (a) are completed as follows: Price and Past Performance,
with past performance being greater in importance than price. Offerors
shall include with their offers a completed copy of the provision at
FAR 52.212-3 "Offeror Representations and Certifications -- Commercial
Items. FAR clause 52.212-4, Contract Terms and Conditions --
Commercial Items is applicable to this solicitation. FAR clause
52.212-5, Contract Terms and Conditions Required to Implement Statutes
or Executive Orders -- Commercial Items is applicable and the
following clauses are checked: 52.222-21, "Prohibition of Segregated
Facilities"; 52.222-26, "Equal Opportunity"; 52.222-35, Affirmative
Action for Disabled Veterans and Veterans of the Vietnam Era";
52.222-36, Affirmative Action for Workers with Disabilities";
52.222-37, "Employment Reports on Disabled Veterans and Veterans of the
Vietnam Era"; 52.232-33, "Payment by Electronic Funds Transfer --
Central Contractor Registration"; and 52.222-41 Service Contract Act of
1965, as amended (41 U.S.C. 351, et seq.). DFARS clause 252.204-7004,
"Required Central Contractor Registration" (CCR) is applicable to this
