494-5828 The VA Desert Pacific Healthcare Network (VISN 22), Network
Business Center (NBC), Long Beach, CA, is seeking to establish a
turn-key Capitated Rate Outpatient Primary Care Clinic (Community-Based
Outpatient Clinic) presence in Hollywood, CA for the VA Greater Los
Angeles Healthcare System (VAGLAHS) within the following area
boundaries: North: Sunset Blvd South: I-10 (Santa Monica Freeway) East:
I-5 and I-110 (Golden State and Harbor Freeway) West: La Cienega Blvd
The qualified (see note) contractor shall provide all clinical space
and outpatient primary care services to include professional fees,
routine phlebotomy and initial specimen processing, radiology technical
procedures, prescribing medications, and EKGs as clinically indicated
at an all inclusive rate. Patients are to have scheduled follow-up
clinic visits that shall be scheduled by the qualified contractor
utilizing the VAGLAHS computer system. VAGLAHS shall provide training
for proper utilization of the scheduling modules. The qualified
contractor may see new referrals or unscheduled patients, under the
contract, once eligibility is established, when under tentative
eligibility as authorized by the VA, or when, upon consultation with
the VA, approval is given. A contact person at the VAGLAHS shall be
established. The Clinic schedule shall be established by and based on
the VA's workweek Monday through Friday, 8:00 a.m. to 4:30 p.m. This
shall allow the VAGLAHS to answer any questions and give guidance on
problems that may arise. Designated contact telephone numbers shall be
provided from the parent VAGLA facility for both Clinic staff and
VAGLAHS patients to use. There shall be no clinic appointment on
Federal Holidays. Primary care is generally defined as the delivery and
management of primary and preventative medical care and the continuity
of that care for all veteran beneficiaries. Population of enrollees is
estimated to be 400 with a growth potential to 2,000. The qualified
contractor would provide clinical providers, medical facilities for
Primary Care, medical equipment and supplies and all administrative
functions to fulfill all levels and ranges of care expected for an
initial one-year period and four (4) subsequent one-year option
renewals. All requests for a solicitation package shall be addressed to
the Contracting Officer in writing using your company letterhead and
received by 4:00 p.m. PDST on May 5, 2000. Fax requests for a
solicitation package shall also be accepted at NBC Fax number (562)
494-5828. The company letterhead must contain a company contact person,
telephone and fax numbers, and complete address. Telephone requests for
a solicitation package shall not be accepted. The anticipated
solicitation issue date is on or about May 15, 2000. The Standard
Industrial Classification (SIC) Code is 8011. Small business and small
disadvantaged businesses are highly encouraged to participate. VA
shall not be responsible for any cost associated with your request for
this solicitation. Note: All usage of the term "qualified" in
reference to personnel and procedures includes reference to a level of
service consistent with current requirements to meet and maintain
accreditation by the Joint Commission for Accreditation of Hospital
Organizations (JCAHO) and that fully meet the standards established by
