Contract costs will be on a per encounter basis and will have an contractual total cap of $150,000.00, which will be the term of the contract when reached or one calendar year from date of award, whichever occurs first. Proposals submitted in response to this solicitation will be evaluated in accordance with the following factors: 1. Past Performance is the single most significant factor, followed by; 2. Qualifications and Experience of Employees; 3. Price; 4. Quality Control Plan; 5. Organizational Structure, Facility Size, and Equipment. All evaluation factors, other than cost, when combined, are significantly more important than cost.
