Q -- RADIOLOGY INTERPRETATION SERVICES SOL RFQ 622-22-00 DUE 052600 POC Annette Gaessler (615) 867-6093 This is a combined synopsis/solicitation for commercial service prepared in accordance with the format in FAR Subpart 12.6 and 13.5 as supplemented, with the additional information included in this notice. This announcement constitutes the only solicitation; This is a Request for Quote and a written solicitation will not be issued. Solicitation number RFQ 622-22-00; award will be made using the simplified acquisition procedures. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 97-16. This acquisition is unrestricted. The standard industrial classification code is (SIC) 809a9 REQUIREMENT: 1. The Radiologist is responsible for providing overflow radiographic interpretation services for the Alvin C. York VA Medical Center, 3400 Lebanon Pike, Murfreesboro, Tennessee, Monday through Friday. The services will include reading and interpreting X-ray films for the Alvin C. York VA Medical Center, Radiology Service. Services will be required for an estimated 50-70 studies per week. Based on the foregoing estimated studies this equates to an estimated 200-275 studies per month that will be read, interpreted, and dictated. Services are to be billed by the study. 2. The Contractor shall provide all equipment, supplies, space and personnel, including the services of a Board Certified Radiologist, for interpretation of films. Contractor's facility must comply with Joint Commission Accreditation of Healthcare Organizations (JCAHO) standards. 3. The services to be performed by the contractor will be performed in accordance with VA policies and procedures and the regulations of the medical staff by laws of the VA facility. The services to be performed by the contractor will be under the direction of Chief, Radiology Service. The Contractor shall be responsible for assuring that radiology reports are completed in a timely fashion satisfying VHA Directives, Medical Center Policies/Memorandums and JCAHO Standards. 4. The VA Medical Center will provide the following: a. Radiographic studies will be performed by VA personnel.. b. The VA Medical Center will provide Radiology (X-ray films) for interpretation to the Contractor. c. Transportation beyond Murfreesboro City limits is the responsibility of the Contractor. 5. Billings will be made in arrears for services rendered. The Alvin C. York VA Medical Center, Murfreesboro, Tennessee, shall establish and maintain a record keeping system that will record the studies interpreted by the contractor's employee(s). This contract requires the Contractor's employee, who meets all of the requirements of the contract, to perform the work specified herein. Any complications will be recorded on a mutually agreed form. Any incidence of Contractor noncompliance as evidenced by the monitoring procedures shall be forwarded immediately to the Contracting Officer. 6. This contract will be for a period of one year, with an option to extend for up to 1 additional one year period. 7. QUALIFICATIONS: Personnel assigned by the Contractor to perform the services covered by this contract shall be licensed in a State, Territory, or Commonwealth of the United States or the District of Columbia. All licenses held by the personnel working on this contract shall be full and unrestricted licenses. The qualifications of such personnel shall also be subject to review by the Chief of Staff and approval by the VA Facility Director. Physicians assigned by the Contractor to perform the services covered by this contract shall be US Citizens and credentialed and privileged in accordance with VA regulations set forth in VA Circular 10-91-070, Credentialing and Privileging of Physicians and Dentists. Physicians must be currently Board Certified in Radiology by the American Board of Radiology or American Osteopathic Board of Radiology and maintain that certification during the duration of the employment. 8. Work Hours: The services covered by this contract shall be furnished by the contractor as defined herein. The contractor will not be required, except in case of emergency, to furnish such services during off-duty hours. (1) Work hours: Monday through Friday, except National Holidays. (2) National Holidays: The 10 holidays observed by the Federal Government are: New Years Day, Martin Luther Kings Birthday, Presidents Day, Memorial Day, Independence Day, Labor Day, Columbus Day, Veterans Day, Thanksgiving, Christmas AND any other day specifically declared by the President of the United States to be a national holiday. 9. Personnel Policy: The contractor shall be responsible for protecting the personnel furnishing services under this contract. To carry out this responsibility, the contractor shall provide the following for these personnel: workers compensation, professional liability insurance, health examinations, income tax withholding, and social security payments. The parties agree that the contractor, its employees, agents and subcontractors shall not be considered VA employees for any purpose. The responsibility for any leave, including any sick leave orvacation time, is the responsibility of the contractor. 10. Contract Performance Monitoring: Monitoring of contractor shall be demonstrated through log sheets. Chief, Radiology Service, shall be the VA official responsible for verifying contract compliance. After contract award, any incidents of contractor noncompliance as evidenced by the monitoring procedures shall be forwarded immediately to the Contracting Officer. 11. Substitution of Physicians: Substitution of personnel shall be made only as follows -- a. All proposed substitutions must be submitted in writing at least 60 days in advance of proposed substitution to the Contracting Officer. All proposed substitutes shall have the appropriate qualifications. b. The Contracting Officer shall evaluate requests for substitution of personnel and promptly notify the contractor, in writing, of whether the request is approved or disapproved. 12. Contractor shall submit with the quote CV's for all proposed physicians along with a tentative schedule for physician coverage. 13. It is anticipated that award will be made by May 31, 2000 with services starting on or about June 5, 2000. Estimated total studies are 2,700.
