99 -- DELIVERY OF DRY ICE SOL A70241 DUE 050800 POC Ida Cofield, Contract Specialist, 301/827-7156 Fax 301/827-7039 The Food and Drug Administration intends to issue a purchase order for the following: The contractor shall provide and deliver 2500 lbs of Dry Ice in the Rice Pellet form to The National Institute of Health (NIH) Building 29A Loading Dock, 8800 Rockville Pike, Bethesda MD 20894. the contractor shall be responsible for placing the product into ten (10) Government supplied bins located on the dock. the remainder of the dry ice is to be deposited into the government supplied large over flow bin also located on the dock. The contractor shall provide and deliver 1110 lbs of Dry Ice in the Nugget Form to The Nicholson Lane Research Center, 5516 Nicholson Lane, Rockville MD 20895. The contractor shall be resonsible for suppling and dropping off a large bin filled with dry ice in the Nugget form and picking up the emptied large bin outside the building each week. The period of performance for the contract will be from date of award through one (1) year. The delivery schedule shall be as follows: Delivery shallbe every Monday during the hours of 8:30AM and 10:30AM excluding holidays. When a holiday falls on a Monday, the delivery shall be made on the Tuesday following the holiday with the same time frame (between 8:30AM -- 10:30 AM). On Fridays the company will receive a call from the contact at the FDA with the number of pounds of dry ice that will be required for the following Monday at both locations if it is different from the above stated requirements. A delivery ticket must be left with each order and tickets must include weight of delivery. All responsible sources who can provide and meet the above requirement shall submit written quotations fifteen (15) calendar days from the date this notice appears in the CBD. No solicitation will be issued, therefore, such requests for solicitations will be considered non responsive. Evaluation/Award will be based on the price offering the best value to the government. All quotes shall include price(s); FOB point; a poc (name and telephone number if applicable; delivery date; business size and payment terms.
