synopsis/solicitation for commercial items prepared in accordance with
the Federal Acquisition Regulation (FAR) sub part 12.6., as
supplemented with additional information included in this notice. This
procurement is being conducted under the Simplified Acquisition
Procedures. This announcement constitutes the only solicitation;
proposals are being requested and a written solicitation and THERE WILL
BE NO WRITTEN REQUEST FOR QUOTATION ISSUED. The solicitation number for
this Request for Quotation is RFQ-797-FDF3-0073. The solicitation
document and incorporated provisions and clauses are those in effect
through FAC 97-16. This solicitation is issued under Standard
Industrial Code 3829 (measuring and controlling devices, N.E.C.), with
a size standard of 500 employees. The Air Force Medical Logistics
Office has a Brand Name requirement for the Fidler Probe G-5 NSN
6665NCD990826MD, manufactured by Rexon Components, Inc. The instrument
is used for detection of low energy radiation. Item comprises a thin
NAI (TL) scintillator crystal of 5 inches in diameter connected to a 5
inch photomultiplier via an optically efficient light pipe; a
protective cover to prevent accidental rupturing of the entrance
window; and crystal thickness is optimized for the S2/b ratio (signal
to noise) and absorbance for low energy x-rays. Item was selected by
the Air Force after extensive field testing. Quantity of 8, Fidler
Probes G-5, Part number SDB990761-7; Quantity of 2 per unit, Service
and Operators Manuals. Equipment shall be Y2K compliant. Place of
delivery is to 311 LS/LGB, 8006 Chennault Road, Building 1150, Brooks
AFB, Texas 78235-5300. Delivery, shall occur within 45 calendar days
after award of the purchase order. Offerors responding to this
announcement shall provide all information contained in the Federal
Acquisition Regulation (FAR) Clause 52.212-1, Instructions to Offeror
-- Commercial Items. As stated in this clause, the Government intends
to evaluate offers and award a contract without discussions with
offerors. Therefore, the offeror's initial offer should contain the
offeror's best terms from a price and technical standpoint. However,
the Government reserves the right to conduct discussion if later
determined by the Contracting Officer to is necessary. Offerors shall
include a completed copy of provision 52.212-3, Offeror Representations
and Certification -- Commercial Items. An authorized representative of
the offeror must sign these representations and certifications; (4)
The Offeror shall also provide its Commercial and Government Entity
(CAGE) code, Contractor Establishment Code (DUNS number) and Tax
Identification number. The resulting firm fixed price award will
incorporate the requirements of the clause at FAR 52.212-4, Contract
Terms and Conditions -- Commercial Items. Clause 52.212-5, Contract
Terms and Conditions Required to Implement Statutes or Executive Orders
