a combined solicitation/synopsis for commercial items prepared in
accordance with the format in FAR Subpart 12.6, as supplemented with
additional information included in this notice. This announcement
constitutes the only solicitation. Request for Quotations are being
requested and a written solicitation will not be issued. Solicitation
number F38610-00-T-0015 is a Request for Quotation. This solicitation
document and incorporated provisions are those in effect through
Federal Acquisition Circular 97-15. This procurement is 100% set-aside
for small business concerns. The Standard Industrial Code (SIC) is
8059. The small business size standard is $5,000,000. The basic
contract period is 1 Jun -- 30 Sep 00 with four (4) option years. The
offeror shall provide as a response to this solicitation: (1) a resume
detailing relevant training and experience; (2) a list of at least
three recent references; and (3) a completed price list (Attachment 1),
and a completed clause 52.212-3. Proposals may be submitted in hard
copy or electronically, either through email, fax, or mail. Regardless
of method of delivery, it is the offeror's responsibility to ensure
receipt of the proposal by the time and date stated on the
solicitation. SECTION 1 GENERAL 1.1. SCOPE OF WORK. The Contract
Educator shall provide care at 437th Medical Group, Charleston Air
Force Base, also referred to as the _medical treatment facility_ (MTF)
herein. Applicant henceforward referred to, as Contract Educator or
Health Care Educator shall provide all patient and professional staff
education, consultations and reports. Educator implements DoD Health
Care Initiatives such as the Breast Cancer Program. Work includes
developing and monitoring programs, including administrative duties,
education and counseling of medical staff and beneficiaries, database
management, population monitoring, statistical analysis,
epidemiological follow-up, and presenting proposals and results to
professional staff. Oversees health care initiatives in a
tri-serviceenvironment coordinating policies and programs with major
commands and Department of Defense (DoD) Health Services Region 3
(HSR3). Serves on the Breast Cancer Regional Action Team including
local MTF coordinators at other MTFs. Coordinates care of military
beneficiaries with professional health care staff. Coordinates MTF and
regional tracking and assessment programs. Tracks education and
counseling, assessing program improvements and coordinating activities
to assure optimum program effectiveness. Collects data to assure
quality care and intervention. Conducts local military and community
awareness programs to include marketing and presentations at public and
professional meetings. Should be prepared for possible attendance at
out-of-town meetings. Work hours should be flexible with a minimum of
24 and maximum 40 hours per week. Performance shall be according to the
requirements contained in this performance of work statement (PWS) and
current recognized national standards as established by the American
Medical Association AMA), the Joint Commission on Accreditation of
Healthcare Organization (JCAHO), and the American Health Association
(AHA). 1.2. PERSONNEL. 1.2.1 AVAILABILITY. The Contract Health Care
Educator shall be available on a weekly schedule designated by the
overall MTF Breast Cancer Coordinator. Plans are for availability for
40 hours per week between the hours of 7:30 a.m. to 4:30 p.m., Monday
through Friday, excluding federal holidays. Applicants desiring to work
30-40 hours per week may also apply. 1.2.2. DRESS CODE. Contract Health
Care Educator personnel shall present a neat appearance and be easily
recognized as a contract employee. They will comply with identification
procedures required by their specific MTF. 1.2.3. ENGLISH LANGUAGE
REQUIREMENT. Personnel shall read, understand, speak, and write
English. 1.2.4. CONFLICT OF INTEREST. The Contract Health Care Educator
shall not be a person who is an employee of the United States
Government if the employment of that person would create a conflict of
interest. The Contractor shall not be any person who is an employee of
the Department of Defense, either military or civilian, unless such
person seeks and receives approval in accordance with DoD Directive
5500.7 and applicable service regulations. 1.2.5. REQUIREMENTS.
1.2.5.1. A copy of the proposed Contract Health Care Educator_s
professional resume, accompanied by that individual_s sworn affidavit
of the truthfulness of same, indicating experience, training, and
technical expertise in the type of care to be rendered. 1.2.5.2.
EDUCATION AND TRAINING REQUIREMENTS. Attendance required at Annual
Breast Cancer Conference. Must also attend Primary Care Manager Breast
Cancer training course. These training opportunities will be
accomplished at first opportunity available utilizing Breast Cancer
Initiative funds. 1.2.5.3. FORMAL EDUCATION. The applicant must hold a
current license in the state they are employed in a health related
field relevant to the Breast Cancer Program. 1.2.5.4.
LICENSE/REGISTRATION. The applicant shall possess a current, valid,
unrestricted license in their health field in the state they will be
employed. 1.2.5.5. EXPERIENCE. The Contract Health Care Educator shall
have provided health care Services or worked in an associated health
care field a minimum of 12 months within the past 3 years. The
applicant shall be knowledgeable of computer software applications to
include word processing (i.e., Microsoft Word), presentation software
(i.e., PowerPoint), spreadsheets and database experience (i.e., Excel,
Access) and familiarity with CHCS, electronic mail and Internet access
is required. Development of briefings and proficient presentation
skills are required. 1.2.5.6. KNOWLEDGE REQUIRED. Ability to perform
literature searches and supply research findings to health education
programs. The applicant shall have expertise in public speaking and the
ability to communicate effectively. 1.2.5.7. The Contracting Office
shall submit only complete credentials applications for processing. A
copy of initial applications shall be submitted to the MTF Health Care
Integration Office. Incomplete applications (those that do not fully
comply with the requirements) will not be accepted by the Government
and will not be returned. Additionally, proposed applicants will be
available for interview by the MTF during the application process. The
Government reserves the right to limit the number of credential
packages it will evaluate. 1.2.5.8. The Contract Health Care Educator
shall be certified and remain current in Basic Life Support (BLS)
coordinated through the MTF. Provided through space available service
at MTF or at contractor cost. 1.3. DENIAL/TERMINATION of HEALTH CARE
EDUCATOR. The MTF reserves the right to terminate the services of
individual when: it is apparent that their job performance is below
accepted standards, their patient management is deemed to be
consistently substandard, they develop a record of unprofessional
conduct, or when it is in the best interest of the Air Force and its
beneficiaries. 1.4. HEALTH REQUIREMENTS. 1.4.1. The Contract Health
Care Educator shall ensure that he/she is in compliance with
preventive, prophylactic, and follow-up procedures, as well as
infection control and employee health program procedures as applicable
per local MTFs Employee Health Regulations. 1.4.2. Also, as a
condition of employment, all contract personnel who will have exposure
to blood or body fluids, or other potentially infectious materials,
shall be offered Hepatitis B vaccine, or have documented proof of
immunity to Hepatitis B infection IAW local MTF Employee Health
Regulations. 1.4.3. Certification of current physical examination for
each employee. The certification shall contain a signed statement by
the examining physician stating that the applicant is free of any
contagious diseases. Physical examination must be current as of 12
months prior to application for privileges. 1.5. HOURS OF OPERATION.
7:30 a.m. -- 4:30 p.m., Monday through Friday excluding federal
Holidays, with the exception of necessary meetings that may occur after
normal duty hours. 1.6. INDEMNIFICATION AND MEDICAL LIABILITY
INSURANCE. The Contract Care Educator shall provide and maintain
adequate liability insurance coverage consistent with the risks
associated with the performance of all services IAW requirements. 1.7.
ORIENTATION. The Contract Health Care Educator shall participate in
the MTF Orientation procedures for newly assigned personnel to include
regulations specific to their professional specialty, MTF and Air
Force policy and procedures.1.8. CONTINUING MEDICAL EDUCATION (CME)
REQUIREMENTS. Health Care Educators registered or certified by
national/medical associations shall continue to meet the minimum
standards for CME to remain current as prescribed in applicable local
regulations. CME shall be obtained at no additional cost to the
Government and shall be reported to the MTFs overall Breast Cancer
Coordinator annually on the first normal duty day in January for the
previous calendar year. Periodic CME may be conducted at the MTF and
will be available, at no cost, to any health care educator desiring to
attend. SECTION 2 -- GOVERNMENT FURNISHED PROPERTY AND SERVICES 2.1.
GENERAL. The Government will provide the following equipment, supplies,
and services listed below: 2.2. EQUIPMENT. The educator shall have use
of all available government equipment for performing services required
by this contract. 2.3. FORMS. The MTF will provide required Government
forms used in the performance of Services. 2.4. SUPPLIES. The MTF will
provide supplies commonly used in the facility for the management of
patients. Access to the Breast Cancer Program computers will be
available. MTFs will provide workspace, desk, chair, and phone. SECTION
3 -- SPECIFIC TASKS 3.1. GENERAL. The contract Health Care Educator
shall provide oversight of health care initiatives in a tri-service
environment coordinating policies and programs with DoD HSR3. 3.2.
PATIENT LISTS. Patient lists, no matter how developed shall be treated
as confidential information. Information protected by Privacy Act &
Freedom of Information Act, including but not limited to address and
SSNs, shall not be disclosed or released in any way for any use outside
the facility except in accordance with law and applicable regulations.
3.3. PATIENT SENSITIVITY. Contract Health Care Educator shall respect
and maintain the basic rights of patients, demonstrating concern for
personal dignity and human relationships. Educators receiving
complaints validated by the MTFs COTR and Chief of Medical Staff, shall
be subject to counseling and, depending on the nature and severity of
the complaint, separation from performing services under this contract.
3.4. RELEASE OF MEDICAL INFORMATION. The contract Health Care Educator
shall only release medical information obtained during the course of
this contract to other MTF staff involved in the care and treatment of
that individual patient. 3.5. COMMUNICATION. The contract Health Care
Educator shall maintain open and professional communication with
members of the MTF. Complaints validated by the MTFs overall Breast
Cancer Coordinator and Chief of Medical Staff shall be reported in
writing to the Contracting Office for action. Failure of the
Contracting Office to correct validated complaints raised by the MTF
staff will be considered a failure to perform. 3.6. PERFORMANCE
EVALUATION MEETINGS. The overall MTF Breast Cancer Coordinator shall
provide the contract Health Care Educator performance feedback at least
semi-annually, and as often as deemed necessary. The contract educator
may request a meeting with the Chief of Medical Staff when he/she
deems such is necessary. APPLICABLE PUBLICATIONS AND FORMS.
Publications and forms applicable to the performance work statement
(PWS) are listed below. The Contractor is obligated to follow those
publications. These publications are available in the MTF and
maintained by the Government. Supplements or amendments to listed
publications from any organizational level may be issued during the
life of the contract. The contract Health Care Educator shall
immediately implement those changes in publications, which result in a
decrease or no change in the price of and notify the Contracting
Officer (CO) in writing of such change. Should a decrease in contract
price result, the Contractor shall provide a proposal within 30 days of
receipt of the change by the Contractor. The CO and the contract Health
Care Educator shall negotiate the change into the contract under the
provisions of the contract clause entitled _changes_. Failure of the
contract Health Care Educator to submit a price proposal within 30 days
from receipt of the change shall entitle the Government to performance
in accordance with such change at no increase in price. DoD Directive
5500.7, Standards of Conduct, Mar 87. CLINIC INFORMATION 1. Volume of
Patients: The 437th Medical Group estimates that the number of Air
Force beneficiaries in the Charleston AFB area is 25,700. Current
TRICARE Prime enrollment is approximately 12,200 for FY00. 2. Average
Patients per hour: Variable, however most patient visits are during
normal duty hours and may be scheduled by appointment with the
exception of those patients who may already be in the facility for a
medical appointment and referred to as a _walk-in_. 3. Shift
times/length of shift allowed: 0730-1630 Mon-Fri, flexible. 4. Who
makes the schedule?: Contract Health Care Educator shall provide a
tentative schedule for approval by Capt Singleton. 5. How far in
advance is the schedule made?: 4 weeks. 6. What kind of flexibility
does the contractor have with the schedule?: See #3 7. Are there
on-call facilities?: No 8. Is there dictation available?: No 9. Who is
in charge of the military clinic?: Lt Col Robert Manaker, MDOS
Squadron Commander 10. Who does the provider report to at the clinic?:
Capt Singleton 11. If the provider had a non-medical problem, who
would he report it to?: Capt Singleton 12. If the provider had an
administrative issue, who would he report it to?: Capt Singleton CLINIC
DEPARTMENT SUPPORT: 1. Availability and Extentof Ancillary Support
Services: Pharmacy, Radiology, and Laboratory elements comprise our
Ancillary Services. These element_s primary mission is to support the
Medical Operations, Aeromedical/Dental Squadrons. 2. Medical staff
On-Call: NA 3. Clinic Specialties/Subspecialties: Family Practice Team,
Pediatric Team, Flight Medicine Team. Behavioral Sciences Flight is
comprised of: a counseling and guidance center, family advocacy program
and substance abuse program. 4. Consultations from Medical Staff
Members: Referrals for self breast exam education and requests for
other patient related education may come from Medical Staff Members,
patients or other sources. The provision at FAR 52.212-1, "Instructions
to Offerors -- Commercial Items" is applicable to this solicitation.
The provision at FAR 52.212-2 " Evaluation -- Commercial Items" is
applicable to the solicitation and the blanks in (a) are completed as
follows: price, technical capability, and past performance, with
technical capability and past performance when combined being greater
in importance than price. Offerors shall include with their offers a
completed copy of the provision at FAR 52.212-3 "Offeror
Representations and Certifications -- Commercial Items. FAR clause
52.212-4, Contract Terms and Conditions -- Commercial Items is
applicable to this solicitation. FAR clause 52.212-5, Contract Terms
