VA Roseburg Healthcare System, Roseburg, Oregon, requires a contractor to provide audiology services which will include the following: 1) audiogram; 2)hearing aid evaluation/fitting; 3) biaural hearing aid fitting; 4) monaural hearing aid fitting; and 5) earmold. The contractor shall provide all materials and labor to perform the audiology services required by the contract. VA Roseburg Healthcare System plans on awarding a multiple-award contract. One contract per county will be awarded within the following counties: Coos, Curry, Del Norte, Douglas, Lane, Jackson, Josephine, and Klamath. Contract performance will be required from June 1, 2000 to September 30, 2000 with the option to extend the contract an additional six months depending upon availability of funds. Contractor will be required to see all patients within the base term period (June 1, 2000 through September 30, 2000). This solicitation is set aside for 100% small business.
