Veterans Affairs,(VA) Network Business Center (NBC) for Southern Nevada
Healthcare System Las Vegas, Nevada,(VA-SNHS LV., NV) requires a
fixed-price, Non-Personal Services, estimated Requirements, Commercial
Item Contract under authority of 38USC8153 for Community Based
Residential Care, Treatment and Rehabilitative Services for homeless
substance abuse veterans. Incorporated are provisions & clauses from
FAC 97-16 (#76) and VAAR (#21). SIC Code is 8099. SOLICITATION REQUESTS

WITHIN CLARK COUNTY, NEVADA, and WITHIN 20-MILE RADIUS of 1700 Vegas
Drive, Las Vegas, Nevada 89106. Provider facility must be BADA
Certified (Bureau of Alcohol & Drug Abuse) and have JCAHO certification
or maintain equivalent standards. This contract is intended to
coordinate with the VA's currentin-house outpatient clinics which will
continue to provide specific follow-up services to veteran patients
who are in Phase II of this contract program. Contract includes those
things required for patient residence, care and treatment, including
staffing and supplies, which are normally provided to patients in this
type of program, and is to be performed at the Contractor's facility.
The Contractor shall be responsible for the administration and
management of all aspects of this contract service. This includes
responsibility for all contractor employees, any subcontracts, agents,
and anyone acting for, or on behalf, of the contractor, and for the
patient's individual testing, evaluation, care, treatment documentation
and reporting to the VA. ESTIMATED 50 patients per year will be
approved for this program. Contract will have a 12 month term from date
of award, with Government option to exercise 4 additional one year
periods. This contract shall at least consist of the following range of
services: The contractor shall furnish services to both male and female
beneficiaries for whom such care is specifically authorized by the VA
SNHS LV., NV. It is understood that the type of patients to be cared
for under this contract will require care and services over and above
the level of room and board. The contractor shall furnish each veteran
authorized care under this contract with at least the following basic
services as part of the contract rate: a. Board and room. Three meals
a day shall be provided on-site. Number of patients per room shall not
exceed acceptable occupancy limits established by regulating agencies.
Necessary bath kit items shall be provided each patient ( soap,
shampoo, razor, etc ) b. Laundry facilities for residents to do their
own laundry -- including providing detergent, hangers, etc. c. Linens,
blankets, pillows, towels etc of a quality which meet applicable
commercial hotel/motel standards for infection control and fire safety.
d. Dayroom or common room for socializing and recreation. e. Essential
Transportation -- All VA placed patients who are in the Phase II
portion of this contract receiving follow-up appointments with VA
Social Work Service, AND any who require transport to VA ACC or MOFH
for non-emergent medical treatment or scheduled VA appointments --
regardless of Phase level, shall be provided transportation, both to
and from the VA. f. Therapeutic and Rehabilitative Services determined
to be needed by the individual patient in a plan developed by the
contractor with consultation by the patient and the VA case manager.
Services which the contractor must be able to furnish include: (1)
Phase I (2 Weeks) -- Intensive residential treatment a. Didactic
lecture, group and individual therapy directed at stabilization and
sobriety skills. b. Secure, monitor and dispense medications which have
been issued to the Veteran by the VA SNHS. (2) Phase II (6 Weeks) --
Supportive residential care a. Instruction in and assistance with
health and personal hygiene b. Secure, monitor and dispense medications
which have been issued to the Veteran by the VA SNHS. c. Supportive
social services, in collaboration with case managers, VA or other
community resources. d. Individual counseling, including counseling on
self care skills, adaptive coping skills and as appropriate,
vocational rehabilitation counseling, in collaboration with VA or
community resources. e. Assistance in learning and development of
responsible living patterns to achieve a more adaptive level of
psychosocial functioning, upgraded social skills and improved personal
relationships. f. Support for alcohol/drug free lifestyle. g.
Assistance in learning, testing and internalizing knowledge of the
illness/recovery process. The contract facility must have a current
occupancy permit or license required by the authority having
jurisdiction. Must be BADA Certified and must have JCAHO Certification
or maintain equivalent standards. Must comply with OSHA, Department of
Health and National Fire Protection Administration (NFPA) requirements.
Will receive inspection review by VA Safety Officer and team as part of
evaluation process and annually for VA reporting. In accordance with 29
CFR, Part 1925, none of the services called for by this contract shall
be performed in buildings, surroundings, or under any working
conditions provided or controlled by the Contractor, that are deemed
unsanitary or hazardous, or otherwise dangerous to the health and
safety of the Contractor's employees or VA beneficiaries. The minimum
standards for facilities furnishing patient Services under this
contract shall be those listed in the latest editions of 42 CFR (Code
of Federal Regulations) and the Life Safety Code (NFPA 1Ol). Unless
specifically excluded in this contract, the all-inclusive contract per
diem rate established will include the services in this document with
all professional and routine services, staff support, non-emergent
transportation to/from the VA ACC and/or MOFH for medical care and
Social Work appointments, and those supplies normally required, or
normally provided to other patients by the facility, without extra
charge to family or third party. SOLICITATION WILL BE AVAILABLE ON OR
ABOUT 5/11/00. This solicitation is being offered as a Small Business
Set-Aside, HOWEVER, it will be converted to UNRESTRICTED acceptance of
offers without further notice if sufficient response is not received
from small businesses. Therefore, all interested, qualified contractors
