R -- EXAMINE POST-ACUTE CARE OUTCOMES AND COSTS FOR STROKE PATIENTS POC Point of Contact: Cheryl Howe, Contract Specialist, 202- 690-5552 The Department of Health and Human Services intends to negotiate with the University of Colorado Health Sciences Center (UCHSC), Center on Aging, Research Section, Hartford/Jahnigen Geriatric Center on Excellence, Campus Box B179, 3570 East 12th Ave., Suite 300, Denver, CO. 80206, on a sole source basis under the authority of FAR 6.302-1 to expand upon work currently funded by DHHS. The new work will examine post-acute care outcomes and costs for stroke patients who receive post-acute care from: (i) home health-only, and (ii) from multiple providers (i.e., patients discharged from skilled nursing facilities to home health and patients discharged from rehabilitation hospitals/units to home health). The purpose of this study is to enhance the analyses of the relative effectiveness and costs of post-acute care providers and to analyze outcomes and costs associated with patterns of care that include multiple post-acute care providers. The UCHSC is uniquely qualified to perform this work given their extensive research expertise in post-acute care and stroke rehabilitation outcomes. UCHSC's expertise includes two large scale national studies examining outcomes for stroke patients in post-acute care settings across episodes of care and different delivery systems. These studies have resulted in numerous publications relating exclusively to stroke rehabilitation. In an existing contract (DHHS Contract #HHS-100- 97-0010, Delivery Order #5), UCHSC also developed the quality measurement tool that will be used under another study (DHHS Contract #HHS-100-97-0013) to measure outcomes and costs in skilled nursing facilities and rehabilitation hospitals. That measurement tool will also be used for the expanded work that is the subject of this sole source arrangement. In addition, UCHSC is leading the development of the research design and is responsible for all outcome and costs analyses required in DHHS Contract #HHS-100-97-0013. The analyses required under this sole source project will use and expand upon the data collected and analyses performed under DHHS Contract #HHS-100-97-0013. The government has determined that to carry out the work required under this sole source arrangement in the most efficient and cost-effective manner the contractor must demonstrate the ability to work with Mathematica Policy Research (MPR), the prime contractor under HHS-100-97-0013. UCHSC has demonstrated this capacity in the role as a subcontractor to MPR under HHS-100-97- 0013. Any organization capable of performing similar services should submit a statement of capabilities with documentation supporting its ability to meet the Government's requirements as described above.
