Hallmark, Contract Specialist @ 254-771-4575 This is a combined synopsis for a commercial item prepared in accordance with FAR Subpart 12.6 as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotations are being requested and a written solicitation will not be issued. This solicitation is issued as a request for quotation RFQ 674-00-41. The Standard Industrial Code is 3844 and the small business size is 500 employees. The solicitation document provides incorporated clauses that are in effect through Federal Acquisition Circular 97-17. The VA Medical Center intends to negotiate a sole source procurement with MRI Devices Corp., 1515 Paramount Dr., Ste. A, Waukesha, WI 53186, for an Echo Speed Neurovascular Array Coil, Product No. 800007, as an accessory designed to capture data from a Seimens Vision Model 1.5T MRI System. This array coil is an uniquely constructed array coil specifically developed for the Seimens Vision Model 1.5T MRI System that captures signals and computer codes for patient testing. Polarity of the array coil system's magnet should be specified as normal polarity manufactured solely by MRI Devices Corp. Therefore, any like array coil that does not match Seimens Vision Model 1.5T MRI System could cause breakdown, malfunction of the testing, and cause the system to become inoperable. Warranty provides the Y2K MRI device be free of defects in material or workmanship for a period of three years as well as replacement guaranteed within 48 hours after receipt of defective parts. If submitting a quote on brand name equal, adequate technical information shall be submitted with the quote for the Government's complete evaluation. Failure to submit adequate technical information shall include but not be limited to technical specifications, product availability, product picture, product features, prices, etc. Quotations shall be submitted on an "all or none" basis. Partial quotations shall be rejected. All quotations state delivery time, F.O.B., F.O.B. point, shipping weightand cost, if any. Offerors shall provide a complete set of provisions with quotation found at 52.212-3, Offeror Representation and Certifications -- Commercial Items, with its offer.
