Request For Quotation RFQ 573-41-00 is advertised as unrestricted with full and open competition. SIC: 8082, size standard of $5 million. The contractor shall furnish all necessary equipment, supplies, supervision and qualified competent personnel to perform home oxygen therapy services for Veterans that meet the eligibility requirements of the North Florida/South Georgia Veterans Health System, Department of Veterans Affairs VA Medical Center, Gainesville, FL. The awarded contractor shall be accredited by JCAHO. The VA intends to award a single contract. The contract base period will be for one year from date of award, with four (4) one year options for renewal. Total duration of the contract will not exceed 5 years. The solicitation will issue on June 14, 2000. The closing date for all quotations is July 14, 2000 at 2:00pm local time. Requests for copies of this solicitation must be in writing and should be received by the issuing agency no later than 15 calendar days from the date this notice is published.
