Q -- RADIOLOGY SERVICES SOL RFQ 573-44-00 DUE 053100 POC Donna Nelson, 352-374-6086 (1)This is a combined synopsis/solicitation for commercial services prepared in accordance with the format in the Federal Acquisition Regulation (FAR) Subpart 12.6, as supplemented with additional information included in the notice in accordance with 38 U.S.C 8153, Sharing of Health Care Resources with non-VA entities. This announcement constitutes the only solicitation. Proposals are being requested and written solicitations will not be issued. (2) This solicitation is issued as a Request For Quotation RFQ 573-44-00. (3) The solicitation document and incorporated provisions are those in effect through Federal Acquisition Circular (FAC) 97-16. (4) This solicitation is advertised as full and open competition with SIC: 8011 with a size standard of $ 5 million. The procurement shall be conducted under FAR 13.5 Test Program, simplified acquisition procedures, and a combination of FAR 12, 13, and 15. (5) [Line Item One]:The contractor shall furnish all necessary supervision and two (2) each Board Certified Radiologist services for the Department of Veterans Affairs, North Florida South Georgia Health System. One Radiologist shall be placed at VA Medical Center, Gainesville, Florida with work hours of Monday Friday, working the hours of 8:00am 4:30pm, Monday through Friday, (40 hours per week with no "on-call" services), effective June 19, 2000 through September 30, 2000. Another Radiologist shall be placed at the VA Medical Center, Lake City, Florida with work hours of 8:00am 4:30pm, Monday Friday, with "on-call" services every 3rd week from Monday through Friday, 4:30pm through 8:00am and including 24 hour per day weekend coverage on the third week, effective upon contract award through September 30, 2000. The following FAR provisions apply to this solicitation: 52.212-1 Instructions to Offerors -- Commercial. FAR clause 52.212-1 Instructions to Offerors -- Commercial is hereby include. The following is an addendum to FAR clause 52.212-1; Offerors are advised that proposals may be evaluated without discussions and are encouraged to submit initial offers as their best offer. Award will be based on price, past performance and technical capability. Technical capability and past performance, when combined, are more important than price. Offerors are requested to submit documentation to support the technical capability and past performance of their company and the individual(s) that are referred for placement to perform services. To be responsive to the solicitation offerors shall submit credentials evidencing registration, certification, and licensure. The VA intends to award a single or multiple award contingent upon availability of candidates. Options. The Government intends to make an award based on the best value to the Government by performing a price/technical tradeoff analysis. In accordance with FAR subpart 9.104-1, each offeror is required to provide information on a reasonable number of recent and relevant contracts for the same or similar services, as well as the offerors primary financial institution, to include contract numbers, verifiable points of contact, telephone numbers and any other relevant information which will be used by the Contracting Officer to make a responsibility determination about a prospective offeror. FAR provision 52.213-3 Offeror Representation and Certification -- Commercial Items, shall be completed and submitted with the proposal. The contract base period will be for the period estimated to be approximately from June 1, 2000 through September 30, 2000. The closing date for all proposals is May 31, 2000 at 4:00pm local time.
