The program is designed to demonstrate that community-based programs, with adequate resources, and designed and led by the communities themselves, can reduce the health disparities which exist in infant mortality, deficits in breast and cervical cancer screening and management, cardiovascular diseases (CVD), diabetes, human immunodeficiency virus (HIV) infections/acquired immunodeficiency syndrome (AIDS), and deficits in childhood and/or adult immunizations. The CDC anticipates a single award contract. The contract period will be four years. The solicitation is scheduled to be issued on or about June 10, 2000.
