RFP 670-05-00 will be issued on or about June 30, 2000 for the provision of services at a location in the County of Cortland or County of Tompkins in the State of New York. The Veterans Affairs Healthcare Network Upstate New York (VAHNUNY) desires to establish a contract for Community Based Primary/Preventative Care services including Mental Health services for veterans in this geographic area. The resulting contract will be for a period of one year with two options to commence on or about January 1, 2001. The Contractor will be responsible to furnish medical facilities, clinical providers, equipment, supplies, routine radiology and administrative functions to fulfill the level of care specified in the RFP.
