The period of performance is changed to reflect the services will be required for an estimated base period of 4-6 months with one option to extend for up to 6 more months. More than one place of performance to complete the examinations is desired. This solicitation is issued to decrease the backlog of VA Optometry Services required. There are an estimated 600 patients requiring services. The services required is changed to only include: Ocular examination (to include but not limited to) history of patient including present complaints, previous eye disorders, family history, general illnesses (present and past), medications, and allergies; Visual Acuity including distant and near with and without correction; Color Vision Testing; Anterior Segment Exam; Intraocular Pressure for glaucoma evaluation; Opthalmoscopy of fundi to evaluate patient for cataracts, diabetic changes, optic nerve, macular changes, retinal problems or vascular changes; and refraction for glass prescriptions. The solicitation will be issued as a Commercial Item under FAR Part 13. It is anticipated the solicitation will be issued June 12, 2000 or earlier and will close approximately June 28, 2000.
