This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation. A quote is being requested and a written solicitation will not be issued. Solicitation number F34612-00-T0046 is hereby issued as a Request for Quote. The incorporated provisions and clauses are those in effect through Federal Acquisition Circular 97-17. This acquisition is restricted and is under Standard Industrial Classification code 7342, with a small business size standard of $5,000,000.00 1.The contractor shall provide all plant, labor, travel, material, supervision, and administration necessary to exterminate termites in designated residential units and commercial/industrial facilities on Altus AFB OK. 2. The work to be accomplished consists, in general, of the following principal items. This does not mean that the work consists only of these items, but that it consists of these principal items along with all incidental minor work which becomes necessary to complete in order to accomplish the overall project in a satisfactory and workmanlike manner meeting the approval of the Contracting Officer. 2.1 Certification: All applications of termiticides shall be accomplished by certified and licensed state of Oklahoma and in the U.S. Environmental Protection Agency (EPA) pesticide applicator category. Submit all certifications and licenses to the contracting officer, through the pest management office of civil engineering, for approval. 2.2 Chemicals: All termiticides will be contractor furnished and stored off Altus AFB. Termiticides used in the execution of work shall have the approval, in writing, of the pest management office of civil engineering. The contractor will provide termiticide contents label and "MSDS" for each chemical proposed for use. 2.3 Schedules: The contractor shall submit a written schedule to cover all termiticide operations. The contractor shall also notify each housing occupant, in writing, a minimum of 24 hours in advance of the date and time their quarters are to be subjected to termiticide operations. 2.4 Reports: The contractor will report all termiticide operations to the pest management office. This report shall cover date, time, site/address/facility, termiticide trade name, EPA registration number, chemical composition/formulation, concentration of original and diluted material, application rate and method of application, area/volume treated, certified/licensed applicator's name with signature, and a signature of contractor's designated representative. Actual formatting of this report is at the contractor's discretion. 2.5 Waste: The contractor will properly dispose of all wastes off Altus AFB. 2.6 Penetrations: The contractor is responsible for providing all penetration points for the application of termiticides; to include removal of carpet and other floor finishes. Upon completion, the contractor shall replace all flooring materials removed and repair all drill holes and penetrations made during execution of the work. Repairs shall be to the satisfaction of the contracting officer. 2.7 Warranty: The contractor shall furnish a three year written warranty against recurrences of termite infestation. Warranty shall include annual, three each, and inspections with written reports for certification that the infestation has not reoccurred. 3. All work shall be performed in accordance with the manufacturer's "written" specifications, installation instructions or performance procedures. 4. Offers on this acquisition shall be evaluated on the basis of price factors only. The government reserves the right to make award on the initial quote without discussions. Each offeror shall include a completed copy of the provisions at FAR clause 52.212-3, Offeror Representations and Certifications-Commercial Items (FEB 2000) with the quotation.
