synopsis/solicitation for commercial items prepared in accordance with
the format in Subpart 12.6, as supplemented with additional
information included in this notice. This announcement constitutes the
only solicitation; proposals are being requested and a written
solicitation will not be issued. Solicitation 626-00-34-jo is issued as
a Request for Quote (RFQ). This solicitation document and incorporated
provisions and clauses are those in effect through Federal Acquisition
Circular 97-17: FAR 52.212-1 Instructions to Offerors; FAR 52.212-4
Contract Terms and Conditions Commercial Items; FAR 52.212-5, Contract
Terms and conditions Required to Implement Statutes or Executive
Orders- This acquisition is unrestricted. SIC is 3841 and small
business size standard is 500 employees. This requirement is for an
KOMIT Plus Imaging System for Fundus and slit lamps with three chip
progression scan camera (interlaced not acceptable). This system
software must be capable of operating under a Windows 95/98 or NT
platform and be fully upgradeable to future CCD technologies with
minimal or no hardware changes. Must include software for retinal
camera imaging and slit lamp imaging, network card, and modem. System
to include a 24" Anthro Cart, Video Adapter, a handicapped accessible
table and a Marco 1147 Adapter. Must be capable, at a minimum, of
(1)saving images to hard disk during acquisition; (2) contact sheet
display with auto contrast; (3) Cup to Disc Ratio by area; (4) create
and print 2,4,6,9 and 36 image collage; (5) archival & search database
of patients with automatic backup; (6) archive to CD-ROM; (7) view
multiple patient files simultaneously; (8) colorized merging of pre &
post procedure images; (9) export images to standard file types; (10)
annotation with test, circle or freehand for patient education; (11)
image enhancement using convolution operation for viewing fine detail;
(12) Cine Animation function for creating a "movie" of pathology
progression over time; and (13) merge anytwo images with automatic
adjustment for angle and orientation. Minimum of one year warranty.
Delivery is to be made to the Logistics Manager, VA Medical Center,
1310 24th Avenue South, Nashville, TN, within 30 days after receipt of
order. Acceptance and Inspection at VA Medical Center, Nashville, TN.
FOB Destination. Companies wishing to respond to this solicitation
should provide this office with the following: (1) A price quote on
company letterhead for the requested item showing solicitation number,
unit price, extended price, FOB, prompt payment terms and correct
remittance address along with a completed copy of FAR Clause 52.212-3,
Offeror Representations and Certifications-Commercial Items. ( Those
companies not in possession of the clause in full text may contact this
office and a copy will be faxed to you); (2) In addition the bidder
shall furnish as part of its bid all descriptive literature and
technical data sheets necessary for the purchasing activity to
determine whether the product offered meets the minimum salient
characteristics/requirements of the solicitation. Failure to provide
this information will cause your bid to be rejected as nonresponsive.
Award of Best Value for the government contract will be based on
ability to meets the requirement specified above, evaluation by end
user, pricing, delivery period, and warranty. Offers are due by close
