This is a combined synopsis/solicitation for Cancer Registry service prepared in accordance with FAR Subpart 12.6 and 13.5 (current as of FAC 97-16) for commercial items, as supplemented with the information included in this announcement. This announcement constitutes the only solicitation; a written solicitation will not be issued. Award will be made using the simplified acquisition procedures found in FAR 13.5. This acquisition is set aside 100% for small business. The standard industrial classification code (SIC) is 8999. 1. Contractor shall provide a credentialed cancer registrar to abstract cancer cases necessary for VA Puget Sound Health Care System (VAPSHCH) to maintain approval by the Commission on Cancer and comply with VHA Directive 97-050. Work product produced in performance of the contract shall be the exclusive property of VA. Contractor responsibilities include (a) records maintenance, including completion of abstraction; (b) Case abstracting and coding; (c) Computer data entry of all cases; (d) Meeting reporting requirements of statewide cancer registry and other regulatory agencies as required; (e) Maintaining credentials through the National Cancer Registrar's Association; (f) Maintaining a cancer database that can be used to evaluate the efficacy of cancer therapy in the region as well as provide a base for epidemiological studies; and (g) Maintaining patient and records confidentiality in accordance with the Privacy Act of 1974, complying with the terms of VA computer access security agreement, and other applicable government regulations. 2. Billings will be submitted monthly in arrears. The VAPSHCS Contracting Officer's Technical Representative (COTR), will establish and maintain a record keeping system that will record the work performed by the contractor. 3. This contract will be for a period of one year, with two additional option years. 4. Work Hours and Location: At the contractor's option, work may be performed on-site at VAPSHCS, utilizing VA computer, phone, fax and copier. VA work hours are Monday through Friday, 8:00am to 4:30, excluding designated National Holidays. If contractor performs work at its own secure facility, the contractor may set any work hours necessary to perform this work; however, overtime pay is the responsibility of the contractor, and will not be subject to additional compensation from VA under this contract. 5. Personnel Policy: Contractor shall be responsible for protecting the personnel furnishing services under this contract. To carry out this responsibility, contractor shall provide the following as applicable: workers compensation, professional liability insurance, health examinations, income tax withholding, and social security payments. Holiday pay, sick leave, vacation and any other fringe benefits are the responsibility of the contractor. The parties agree that the contractor, its employees, agents and subcontractors shall not be considered VA employees for any purpose. 6. Contract Performance Monitoring: The VAPSHCS COTR will monitor contract performance via audit of completed and in-progress records. After contract award, any incidents of contractor noncompliance as evidenced by the monitoring procedures shall be forwarded immediately to the Contracting Officer. Any violation of patient privacy or VA computer access policies may be grounds for immediate termination. 7. Vendors must submit a price quote for the following estimated quantities, for each of the three contract years: a. Abstract Cases -- 675 cases/year; (b) Case Consultation -- 144 hours/year; (c) Patient Care Evaluation Studies -- 50/year; (d) Consultation fee -- 48 hours/month. Quotes must be in writing and accompanied by a completed Representations and Certifications or they will be rejected. Facsimile quotes and/or oral quotes will not be accepted. Anticipated award date is on or about 6/30/00. A single award will be made to the offeror whose quote conforms to the requirements of the solicitation, best meets the minimum needs of the Government and has the lowest price, all factors being considered.
