POTENTIAL SOURCES SOUGHT was submitted to CBD on June 1, 2000 and
contained an error. The due date for 254's and 255's was not completed.
Therefore, this modification is issued to correct that deficiency.
Furnish A/E Services for Project No. 509-314, Clinical Improvements
SCIU, Downtown Division, 800 Bailie Drive, Augusta, Georgia. Project
involves the addition of approximately 14,000 SF and the renovation of
approximately 4,000 SF to the existing Spinal Cord Injury Unit in
Building 801, Downtown Division. The new space will include additional
outpatient clinical space and associated administrative areas.
Existing space will be renovated to improve current operations and
provide training space. SELECTION CRITERIA: A. Firms must demonstrate
experience with hospital design, especially outpatient clinical units.
Experience with specialty care (Spinal Cord Injury) units is desirable
and preferred. B. Experience with government agencies, especially
Department of Veterans Affairs, while not required is desired. C. Firms
must have a registered architect on staff. This project will require
professional services of a registered Civil/Structural engineer,
Electrical engineer, Mechanical engineer, and Fire Protection engineer.
These are not required to be on staff. D. Firms must be able to provide
all construction drawings in AutoCAD R14 or AutoCAD 2000 format. E.
Firms must be able to provide all specifications in Microsoft Word
(preferably Word 97) format. F. Firms must demonstrate the ability to
complete required work within established time frame and budget. G.
Area of consideration is limited to a 150 mile radius of Augusta,
Georgia. SELECTION: A. Selected VA Engineering and Contracting staff
will review all submitted experience and qualification (A/E 254s/255s)
packages to determine the highest qualified firms (short listing). B.
All short listed firms will be invited to make a presentation (45 min.
to 1 Hr.) to the VA selection board. C. The VA selection board will
rank and rate allshort listed firms. D. A fee proposal will be
requested from the top ranked firm. E. If the VA and the top ranked
firm cannot reach an agreement concerning the cost to perform this
work; a fee proposal will be requested from the second ranked firm. All
interested firms shall submit SF 254, A/E and Related Services
Questionnaire, and SF-255, A/E Related Services for Specific Projects,
to the Contracting Officer (111A), VA Medical Center, One Freedom Way,
Augusta, Georgia 30904-6285, no later than July 5, 2000. ESTIMATED
CONSTRUCTION COST RANGE: $2,000,000.00 to $5,000,000.00. This
