synopsis/solicitation for commercial items prepared in accordance with
the format in Subpart 12.6, as supplemented with additional
information included in this notice. The announcement constitutes the
only solicitation; proposals are being requested and a written
solicitation will not be issued. (ii) This solicitation,
030-534-0-0010, is issued as a request for quotation (RFQ). (iii) This
solicitation document and incorporated provisions and clauses are
those in effect through Federal Acquisition Circular 97.17. (iv) The
goals of the Running and Fitness Camps series are uncomplicated. They
will provide a fun, safe, drug-free environment for the children to
experience the joy of physical activity. They will provide successful
Indian high school and college role models for the children to identify
with, and give them basic skills in stretching, walking, running and
nutrition. Goal is to encourage the youth to make a commitment to a
healthy lifestyle. The camps should be facilitatedby Navajo and Native
American role models especially successful college and high school
athletes. The contractor should also hold a leadership training in the
summer for local Navajo and Native American high school runners. One
of the goals of the training is to help the runners recognize their
potential as role models in their home communities. One way to
accomplish this goal is to have graduates of the leadership training
serve as assistants in the Running and Fitness Camps for children. (vi)
A. Tasks [i] Vendor will be responsible for planning, implementing, and
evaluating a series of running and fitness camps/clinics in Navajo
communities during summer 2000 including but not limited to: 1.
Identify all trainers, facilitators, guest speakers, and assistants for
the camps and making arrangements for their travel/meals. 2. Hold a
leadership training in the summer for local Navajo and Native American
high school runners. 3. Identify locations and secure all clearances
required. 4. Develop and provide all training materials. 5. Advertise
the events/agenda for the clinic/camp. 6. Coordinate a community fun
run event in conjunction with the kids camp. 7. Coordinate all efforts
with local NAIHS health promotion staff, and the NAIHS Health
Promotion Program. 8. Evaluate all camps/clinics including responses
from participants and parents. 9. Clinics should be large enough, and
staffed well enough to provide a chance to 100 students per site to
participate. 1.) Total number of clinic sites in 2000 = 12. 2.) Total
number of clinic days in 2000 = 18. [ii] Vendor will also assist with
other activities in support of fitness promotion on the Navajo Nation,
including the development of promotional campaigns, incentives, and
local running /walking clubs. B. Reporting Requirements: The Contractor
shall provide summary reports for each camp/clinic to Navajo Area
Health Promotion Program Director including overall participation
rates, summaries of group and individual activities, feedback from
students and parents, and recommendations for next year's programs. C.
Deliverables: Contractor shall provide services as outlined above.
Total number of clinic/camp days to be provided in summer 2000 is 18
days at 12 sites. D. Schedule: Upon award of contract, vendor will
initiate project and carry it out over the following two (2) months.
Place of delivery of monthly summary invoice and final evaluation
summary: Office of Community Health Services, Shiprock Service Unit,
P.O. Box 160, Shiprock, NM 87420. Method of delivery shall be in person
by mail or federal express. E. Period of Performance: Project is
estimated to take two (2) months. F. Government property: No government
property will be involved in the carrying out of this contract. (vii)
Government proposed delivery date is July, 2000 through September 2000.
FOB for this RFQ is Destination. Performance of services shall be
accomplished between 7:30AM and 4:30PM MDST Monday through Friday.
(viii) Offeror must comply with all instructions contained in FAR
52.212-1, Instructions to Offerors-Commercial. (ix) The following
additional FAR provisions and clauses apply: FAR 52.212-2,
Evaluation-Commercial Items, The Government will award a contract
resulting from this solicitation to the responsible offeror whose offer
is conforming to the solicitation will be most advantageous to the
Government, price and other factors considered. The following factors
shall be used to evaluate offerors: 1.) Ability to implement community
running / fitness camps at the local community level, a) 0 points no
previous fitness camp experience 25 points b) 25 points -- Extensive
experience in implementing local, community-based fitness and running
camps for Navajo children. 2) Ability to provide services on-site in
Navajo communities 15 points, a) 0 points no previous experience
working with Navajo communities. b) 15 points able to provide service
on site at all 12 Navajo communities in a timely fashion. 3)
Demonstrated experience in Native American youth leadership training
and ability to recruit, train, and employ local Navajo and Native
American role models and athletes as instructors/facilitators
/assistants for community fitness/running camps 20 points, a) 0 points
No experience in working with Navajo or Native American athletes as
employees or instructors. b) 20 points Extensive experience in working
with Navajo or Native American athletes as employees and instructors.
4) Methodology and Approach -- 20 points, a) Soundness, feasibility,
and practicality of the proposed approach and work plan to provide the
series of camps/clinics/ and events. b) Recognition of and plan for
overcoming the difficulties involved in implementing the series of
camps/clinics. 5) Understanding of the Project -- 20 points, a)
Perception of the needs and purposes of this project as demonstrated
through a knowledge and understanding of current health and fitness
promotion efforts for native people, past efforts, and the clarity and
practicality of the offeror's proposed implementation plans. Price
will be evaluated to determine fairness, completeness, and
reasonableness as it relates to the services offered. (x) FAR 52.212-3,
Offeror Representation and Certifications-Commercial Items, a completed
copy of this provision shall be submitted with the offer. The following
provision applies: FAR 52.212-4, Contract Terms and Conditions-
