all labor, parts, materials, transportation, supervision required to
remove existing carpet and install new carpeting. Contractor shall be
responsible for moving all furniture. **** **** **** **** **** ****
**** **** Performance Work Statement For Carpet Installation in
Building 1669 (Base Chapel) 1. GENERAL: The contractor shall furnish
and install carpeting and rubber base in accordance with manufacture
recommendations. The contractor shall perform all services to the
standards set forth in this contract and in accordance with Federal,
State and local regulations. 2. WORK SITE: Work is to be performed at
Kelly AFB Building 1669 (Drawings available on request). 2.1 All
equipment required for the performance of this work shall be provided
and maintained by the contractor. All equipment shall be kept in good
working order and will meet all the current base safety requirements
for the trade. Personnel safety shall be observed at all times in
accordance with OSHA and AFOSH directives. 2.2 The contractor or
assigned representative shall be in contract with Mr. Earnie Pieniazek
(210-925-8704), at all times, when performing work on Kelly AFB. 3.
SPECIFIC TASKS AND SERVICES: The contractor shall provide the following
initial services: Furnish all plant, labor, materials, equipment and
supervision necessary to remove and reinstall new carpeting and rubber
base in hallways and office areas to include the movement of
furniture. 3.1 Carpeting to be installed is Kraus "Anchorage or
Montreux" carpet and Roppe 4" rubber base or equal substitute. 3.2
MEASUREMENTS: The contractor shall verify all measurements at the work
site and shall be responsible for dimensions, fittings, alterations,
and the proper attachments. 3.3 CLEAN UP: Upon completion of workday,
the contractor shall leave the work area in a neat and workman like
manner. 3.4 REMOVAL AND DISPOSAL: The contractor is responsible for
removal of all debris and any dead birds resulting from the work
activities, debris shall be removed to an off-base site. Utilizing base
dumpsters for disposal of job site refuse by the contractor is
prohibited. Use of base dumpster(s) shall result in action taken
against the contractor or responsible party. 3.5 INSPECTION: Upon
completion, Mr. Pieniazek or authorized government representative shall
perform inspection. **** **** **** **** **** **** **** **** ADDITIONAL
INFORMATION: Request for Quotation (RFQ) Packages shall be available
on 8 Jun 00. RFQ's must be returned by close of business on 15 Jun 00.

communicate concerns, if any, concerning this acquisition. If your
concerns are not satisfied by the contracting officer, an Ombudsman has
been appointed to hear concerns from offerors or potential offerors
during the proposal development phase of an acquisition. The Ombudsman
does not diminish the authority of the program director or Contracting
Officer, but communicates Contractor concerns, issues, disagreements,
and recommendations to the appropriate Government personnel. When
requested, the Ombudsman shall maintain strict confidentially as to the
source of the concern. The Ombudsman does not participate in the
evaluation of proposals or in the source selection process. Interested
