synopsis/solicitation for commercial items prepared in accordance with
the format in Subpart 12.6 as supplemented with additional information
contained in this notice. This procurement is being conducted under
Simplified Acquisition Procedures in accordance with FAR Part 13. This
announcement constitutes the only solicitation a written solicitation
WILL NOT be issued. The solicitation is issued as a Request for
Quotation (RFQ). The solicitation document and incorporated provisions
and clauses are those in effect through FAC 97-17. The VA intends to
award a firm-fix priced, non-personal services contract for Health Care
Resources in accordance with Public Law 104-262 and the authority of
Title 38 USC Section 8153. WJBD Veterans Affairs Medical Center (VAMC)
plans to negotiate on a sole source basis with SC Medical Endoscopy
Center, 1735 Taylor Street, Columbia, SC 29201. The Government intends
to solicit and negotiate with only one source under the authority of
FAR 6.302-1. Interested persons may identify their interest and
capabilities to respond to the requirement or submit proposals. This
notice of intent is not a request for competitive proposals. A
determination by the Government not to compete this contract based upon
responses to this notice is solely the discretion of the Government.
Information received will be considered solely for the purpose of
determining whether to conduct a competitive procurement. The SIC Code
is 8011 and the size standard is $5.0 million. Solicitation package
will be available o/a 6/7/00. The contractor shall perform
approximately 400 each Sigmoidoscopys and Colonoscopys procedures on
entitled VA beneficiaries of the WJBD VAMC from July 1, 2000 through
September 30, 2000. Contractor shall provide a per capitated rate per
procedure. The Contractor will be required to perform the following
procedures: Sigmoidoscopys, CPT Codes: 45330, 45331, 45333, 45334,
45385 and Colonoscopys, CPT Codes: 45378, 45380, 45382, 45383, 45384 in
accordance with all terms, conditions and provisions. Cost should
represent and "all inclusive" reimbursement for these procedures.
STATEMENT OF WORK: (A) The contractor shall provide physicians who are
board certified or board eligible in Gastroenterology. (B) The
procedures shall be provided at the office of the contractor. (C)
Contractor shall provide a dictated report of all the procedures on
disk that will be read by the VAMC. (D) VA will provide to the
Contractor a list of names, addresses, social security numbers and
telephone numbers of the patients that are to be scheduled for the
procedures. (E) Bowel preps will be mailed to the patient from the
Pharmacy at the VA based on prescription that is written by contracting
physician and brought to the VA by courier service. (F) Appointment and
information letters and preparation instructions will be mailed to the
patient by contractor's office with a map giving directions on how to
get to their office. (G) The VA will provide specimen containers
approved by the VA Laboratory to the contractor. Specimens will be
picked up via courier service from the contractor's office and brought
to the VA Laboratory. The contractor's office staff will notify the
courier service when specimens are to be transported. (H) Pathology lab
results will be printed by VA Staff and faxed to the contractor. (I)
Contractor provided physicians will comply with JCAHO and the
Department of Veterans Affairs (VA) requirements pertaining to patient
treatment, VAMC Quality Assurance requirements, locally accepted
tenets of standards of care, and the Mission of the VA. Non-compliance
of these requirements may result in the revocation of clinical
privileges. (J) The services specified may be changed by written
modification issued by the Contracting Officer. (K) The contractor may
be required to remove any contract physician who is found not to
practice with acceptable medical standards of care for this
institution, violates patient care policies, or commits an unlawful act
or act of patient/staff abuse including sexual harassment. (L) All
physicians shall have credentialing and privileging completed by the VA
before performing procedures under this contract. (M) The parties agree
that the Contractor, its employees, agents and subcontractors shall not
be considered VA employees for any purposes. (N) Payments will be made
monthly in arrears. Contractor shall submit invoices to: Chief, Fiscal
Service (544/04), WJBD Veterans Affair Medical Center, 6439 Garners
Ferry Road, Columbia, SC 29209. Contractor's invoices shall reference
the following information: Contract No., Patient Name, Patient's Social
Security Number, Procedure CPT Code, and Contractor's Charge per CPT
Code. Contractor shall be paid on a per capitated rate per procedure.
(O) No services shall be performed after September 30, 2000. (P) The
Contractor shall comply will all applicable Federal, State and Local
laws, executive orders, rules and regulations applicable to its
performance under this contract. (Q) Upon award of a contract, the
Contracting Officer will appoint a Contracting Officer's Representative
(COR) to assist with the contract monitoring requirements. The
