MEDICAL SURVEILLANCE. Quantity: 1 Job. ; <FILL IN SUPPLEMENTAL

<FILL IN>. Anticipated award date: 5 July 2000. All responsible
sources may submit a proposal which shall be considered. For EDI
transfer, <FILL IN>. Medical surveillance testing to be completed
between 26 September 2000 and 30 October 2000. Quotes should be made on
a per test basis. All numbers are approximate and are not binding. The
Georgia Army National Guard guarantees at least 80% participation of
employees in the program. Georgia Army National Guard will not be
responsible for any special equipment needed by the contractor or for
installation of any equipment. Proposed schedule for testing is to be
provided to the Occupational Health Nurse NLT 28 July 2000 for review
and approval. Testing will only be performed Tuesday through Friday.
Testing will not begin before 0715 and will stop NLT 1700 each day.
Authorization for test can onlybe given by the State Occupational
Health Nurse. An authorization form must accompany each employee at
time of testing. Only test authorized by the Occupational Health Nurse
will be billed to the Georgia Army National Guard. Contractor to
provide all hard copies of test results and a complete master listing
of results in ledger form to the Occupational Health Office NLT 45 days
after all testing has been cocmpleted. Employee will be provided a copy
of all test results along with physician comments. Results to be suend
directly to employee at an address specified by the employee within 2
weeks of testing. Physician to review all test results for accuracy.
Physician shall report any possibly serious abnormal results as soon as
reasonable but not more than 1 week from time of test to the
Occupational Health Nurse. Physician to perform on site, hands-on
examination of employees that are on the respirator program. At the
discretion of the contractor, on site physician examination may be
completed at the time of the van testing or the physician may set up a
seperate visit to the site. Examinations must be completed between 26
September 2000 and 30 October 2000. One consolidated invoice for all
sites should be submitted to the Nurse for the review and payment NLT
30 days after completion of test. Contractor will provide all test
results on a disk that is compatible with the Army National Guard
Occupational Health Monitoring System. VISION SCREENING: Multiphasic
vision screening equipment (Titmus type), able to measure the following
primary visual skills: Visual acuity for distance and near. Lateral
phorias for distance and near. Vertical phorias for distance and near.
Depth Perception. Color Preception. Peripheral Vision. PULMONARY
FUNCTION TEST WITH CLEARANCE: Including as a minimum the determination
of the forced expiratory volume in one second (FEVI) and the forced
vital capacity (FVC). Written clearance by a physician to wear a
respirator. Test to be administered by certified operator only. BLOOD
CHEMISTRY (LEAD AND ZZP) Analysis for lead levels and analysis for zinc
protoporphyrin levels. BLOOD CHEMISTRY PROFILE: To include but not
limited to at least the following: Comprehensive Liver function
profile. Cholestrerol with HDL to total cholesterol ratio. Serum
Ferritin Level. AUDIOGRAM: Audiogram with range of 500 Hz to 6,000 Hz
with a zero reference. Calculations should be without age correction.
Test to be administered by certified operator only. Locations and
