This modification is to the original notice posted June 15, 2000. Change the number of "sessions" from: one per quarter, to: one (1) per week. Provide Optometry services for the inmates incarcerated at Federal Correctional Institution (FCI) Phoenix, Arizona located at 37900 N. 45th Avenue. Services provided will include: diagnostic optometric examinations and; other related procedures as approved by the Federal Bureau of Prisons (FBOP) on-site Clinical Director. Examinations and most work to be performed within the secure perimeter of FCI Phoenix. Work includes keeping related medical records. Work will be performed during four hour "sessions". There will be one "session" scheduled each WEEK. Quoters will be asked to provide a per "session" price for the base year and each option year. The Government intends to award a single firm fixed price contract, however, it reserves the right to make multiple awards. A contract will be awarded for a base year, with four one-year option periods. The base contract period will run from date of award through September 30, 2000. Award of each option year will be contingent upon funds being made available for that year. All Federal Acquisition Regulations (FAR) will apply to this procurement. This will be a "Non-Personal" contract with the work being "Professional" in nature. The contractor may not be directly supervised by a Government employee during performance of the contract. There may not be even the appearance of an employer-employee relationship. All work must be performed in the manner deemed appropriate by the contractor. The contractor must, at all times, perform the work required, independent of direct Government supervision. No individual, contractor, or employee of a contractor, may work more than 30 hours per week. If award is to an individual or individuals who will personally be providing the services, all Department of Justice, Bureau of Prisons security and background requirements must be met, and clearances issued, prior to award of the contract. If the contract will be awarded to a contractor who will provide different individuals for performance, the contract may be awarded prior to clearances being issued. However, all individuals involved in the performance of the contract must meet all Department of Justice, Bureau of Prisons security and background requirements prior to their being allowed to begin performance. This contract is being solicited and awarded under the FAR Test Program for Certain Commercial Items found at FAR Subpart 13.5. All rules for "Quotations" apply, that is, the "quoter" is not bound by the price quoted, but rather by the actual signing of a "contract".
