Hill, (214) 857-0031 1. Introduction: The VANTHCS issues this
solicitation and anticipates the award of a firm fixed price ID/R
contract therefrom for the provision of transcription services
resulting in an accurately and neatly typed record which will serve as
a medico-legal document, the accuracy of which will affect the
continuity of patient care and the VA image and may affect the outcome
of legal issues. 2. Scope of Work: The Contractor shall transcribe,
from DVI, hospital and Social Work Service summaries and highly
technical reports of procedures to include operations, Discharge
Summaries, miscellaneous procedure notes, History and Physical
Examinations. The Contractor shall transcribe all reports in a final
format without access to the patient's medical chart, including
transcription of medical data from any vocal delivery. Because the
VANTHCS is a teaching facility, foreign and regional accented dictation
is often encountered and there is frequent change in dictators due to
rotation of medical residents and interns. The Contractor edits for
stylistic consistency and is required to format, punctuate and spell
correctly, as well as use proper grammar in producing an accurate
medical report that becomes a permanent record for medical legal and
research purposes. The Contractor may be required to change wording to
eliminate idiomatic expressions, provide clarity of expression,
correct grammatical errors and insure text consistency. Any editing
which may possibly change the meaning of the document will be brought
to the attention of the VANTHCS COR for guidance. The Contractor is
also required to maintain confidentiality of patient records. 3.
Contractor Qualifications: The Contractor shall be able to provide, if
requested by the CO, evidence of past performance, in the past
calendar year, of medical transcription services that are similar in
magnitude to those services required in the solicitation. If requested
by the CO, past performance will be evidenced by the Contractor's
provision of references. Those Contractor employees performing
transcribing duties under the terms of an agreement shall have
knowledge of punctuation, spelling, capitalization, grammar, medical
terminology, and arrangement of all material on completion of a
transcribed document. 4. This acquisition is for a commercial item and
has been totally set aside for small business. Electronic copies of
