commercial services prepared in accordance with the format in Subpart
12.6 as supplemented with additional information included in this
notice. This announcement constitutes the only solicitation; quotes are
being requested and a written solicitation will not be issued. The
solicitation is being issued as Request for Quote #565-26-00. The
solicitation document and incorporated provisions and clauses are those
in effect through Federal Acquisition Circular 97-14. The SIC code is
7363 and the small business size standard is $5.0 million. The VA
Medical Center, Fayetteville, N.C. has an urgent need for services of
a CRNA and intends to solicit proposals and award a firm fixed price
contract under the authority of Title 38 USC Section 8153, pending
availability of funds. Contract period will be established from date of
award for the period of 90 days with option to extend. Estimated start
date mid-July. WORK STATEMENT: SURGICAL SERVICE/ANESTHESIA CRNA 1.
Furnish a Certified Registered Nurse Anesthetist, ACLS certified, to
perform all duties and responsibilities of a staff nurse anesthetist.
This CRNA will be responsible to Chief of Surgery and/or the Chief of
Anesthesia Section, who assigns CRNA duties. This CRNA should have
demonstrated clinical training and competence to complete the duties as
listed: (a) administer IV, inhalational and regional anesthesia as
directed by Chief of Surgery to patients under going general,
orthopedic, urological and oral surgery. (b) Monitored anesthesia care
as needed in those patients listed under (a) as well as those
scheduled for MAC for endoscopy, pacemaker insertion, cardioversion and
other diagnostic and therapeutic procedures. (c) Management of cardiac
and respiratory resuscitation. (d) Screening, evaluation and
documentation of any psychosocial and physical condition that could
influence the management of anesthesia and in conjunction with the
attending physician and/or Chief of Surgery determine the patients
status to undergo anesthesia and surgery. (e) Assemble and inspect
necessary equipment gas sources, drugs and supplies needed to cope with
any foreseeable eventuality. (f) Utilize standards for basic intra
operative monitoring. (g) Maintain adequate records as outlined in the
anesthesia policy. (h) Follow Category I risk classification for blood
borne pathogen exposure. (i) Utilize measures to protect self and
patient during periods when radiation is in use and during laser
activity. 2. Professional Tour: 8:00 am until 4:30 pm. Will be on call
after normal duty hours in rotation with other CRNAs. 3. Other
Anesthesia related duties as assigned. 4. CRNA will be required to
provide on call coverage during normal week-days, and after duty hours
in rotation with other CRNAs. CRNA must be able to respond within 20
minutes from receipt of call. Beeper will be provided by the VA. COST
PROPOSAL: Provide all inclusive rate covering all fees, professional
liability insurance, any housing and transportation costs involved
including hourly rate; overtime rate; on call availability and rate.
Personnel assigned by the contractor to perform the services covered by
this contract shall be licensed in a State, Territory, or Commonwealth
of the United States or the District of Columbia. Personnel performing
these services shall be a certified registered nurse anesthetist and
ACLS certified and must meet the credentialing privileging requirements
of the VA Medical Center, Fayetteville, N.C. Services to be performed
by the contractor will be under the direction of the Chief of Staff or
his/her designee at the VA Medical Center, Fayetteville, N.C. The
following information is provided as general information. The
contractor under the terms of the contract shall furnish the work hours
as mutually agreed upon in the work statement. Work hours: Monday
through Friday -- 8:00 a.m. -- 4:30 p.m. National Holiday: The ten
holidays observed by the Federal Government, i.e. New Year's Day,
Martin Luther King's Birthday, Washington's Birthday, Memorial Day,
Independence Day, Labor Day, Columbus Day, Veterans Day, Thanksgiving,
and Christmas Day and any other day specifically declared by the
President of the United States to be a national holiday. Nonworking
hours (normal on-call hours) Monday through Friday, 4:30 p.m. to 8:00
a.m., and all Saturdays and Sundays. Emergency On-Call Hours: 12:00
Midnight Sunday through 12:00 Midnight the following Sunday. Work
performed without prior approval of the Contracting Officer is not
reimbursable. The contractor shall be responsible for protecting the
personnel furnishing services under this contract. To carry out this
responsibility, the contractor shall provide the following for their
personnel: worker's compensation, professional liability insurance,
health examinations, and income tax withholding and social security
payments. The parties agree that such personnel shall not be considered
a VA employee for any purpose and shall be considered employees of the
contractor. Payment for any leave, including sick leave, holiday or
vacation time is the responsibility of the contractor. The following

Evaluation-Commercial Items; Offerors responding to this announcement
shall submit a cost proposal and technical proposal to include the
following evaluation factors company experience, personnel
qualifications to include evidence of current licensure and
credentials/privileges and current CV of candidate, Indemnification and
Medical Liability Insurance certificate, past performance of contracts
of similar type, scope size and complexity that are ongoing or
completed within past three years, references with points of contacts
for contracts identified. Technical and past performance, when
combined, are significantly more important than cost or price. Award
will be made to the offeror who offer conforming to the solicitation
will be most advantageous to the Government, price and other factors
considered. The government intends to evaluate offers and award without
discussions. However, the government reserves the right to conduct
discussions later if determined necessary by the Contracting officer.

Business certification must be submitted with offer. Quotes including
