combined synopsis/solicitation for commercial service prepared in
accordance with the format in FAR Subpart 12.6 and 13.5 as
supplemented, with the additional information included in this notice.
This announcement constitutes the only solicitation; This is a Request
for Quote and a written solicitation will not be issued. Solicitation
number RFQ 667-28-00; award will be made using the simplified
acquisition procedures. This solicitation document and incorporated
provisions and clauses are those in effect through Federal Acquisition
Circular 97-18. This acquisition is unrestricted. The standard
industrial classification (SIC) 7361. THE SCHEDULE 1. Furnish 2
full-time equivalent (FTE) Anesthesiologist to perform the requirements
of this contract 4160 Hr 2. Overtime Rate 100 Hr 3. On Call/Standby
Week Nights 26 Day 4. On Call/Standby Weekends 20 EA An FTE is defined
as a contractors employee providing service 8 hours per day, 40 hours
per week, 52 weeks per year. The above prices are all inclusive to
include travel, lodging, car rental, malpractice and all related
expenses with providing. REQUIREMENT: The Anesthesiologist Services:
Start Date July 31, 2000. 1. The contract resulting from this
solicitation shall be to provide Anesthesiology Services at the Overton
Brooks VAMC, herein called VAMC, 510 East Stoner Avenue, Shreveport,
Louisiana 71101. The contractor agrees, in accordance with the terms
and conditions stated herein, to furnish the services specified and at
the prices specified in this contract. The initial listings of
services to be furnished shall be identified in the schedule. Services
shall include, but are not limited to the following: a. Pre-operative
evaluation of patients. b. Administer anesthetics and conduct support
therapy. c. Undertake post operative therapy referable to
anesthesiology. d. Furnish technical and/or professional direction of
medical students, interns and support personnel employed by VA. e.
Furnish consultations referable to anesthesiology and emergency call
services. 2. The payments for any leave, including sick leave, holiday
pay or vacation time, are the responsibility of the contractor. 3.
Provide technical and professional direction of post-operative care of
patients in Post Anesthesia Care Unit, on Surgical Wards and the
Surgical Intensive Care Unit, in conjunction with the Surgical Staff.
4. Authority: This procurement is executed under Enhanced Sharing
Authority, Public Law 104-262, Title 38 U.S.C. 8153. 5. TERM OF
CONTRACT: a. This contract is effective for twelve months for one FTE
starting on or about July 3, 2000. The contract is subject to the
availability of VA funds. No service shall be performed by the
contractor after September 30, 2000. The Second FTE will be for four
months with the option to extend in one month increment but not to
exceed 12 months total. 6. WORK HOURS: a. Monday through Friday, 7:00
a.m. to 3:30 p.m., eight (8) hours per day, 40 hours per week. Or
Monday through Friday, 7:30 a.m. to 4:00 p.m., eight (8) hours per day,
40 hours per week, as determined by the Department Head, based on case
workload. A 30 minute lunch break is allowed. b. National Holidays:
The ten (10) holidays observed by the Federal Government, i.e., New
Years Day, President's Day, Martin Luther King Jr.'s Birthday, Memorial
Day, Independence Day, Labor Day, Columbus Day, Veterans Day,
Thanksgiving Day, and Christmas Day. 7. QUALIFICATIONS: Personnel under
this contract shall be licensed in a State, Territory, or Commonwealth
of the United States or the District of Columbia and Board Certified.
The qualifications of such personnel shall also be subject to review
by the VAMC Chief of Staff and approval by the VAMC Director. Personnel
assigned under this contract shall be Board Certified or Board
eligible. Board eligible candidates must not exceed 4 years from the
date they became eligible. Submit copies of the individual credentials
with your quote. 8. PAYMENTS: Sums due for actual work done, the
Contractor will be paid monthly, in arrears, upon receipt of a properly
prepared invoice FAR 52.232-25. The Contractor shall submit all
invoices to the following address: Chief Fiscal Service (04) Overton
Brooks VA Medical Center 510 East Stoner Ave. Shreveport, LA 71101. 9.
PERSONNEL POLICY: The contractor shall be responsible for providing:
-- Health examinations. -- Worker's compensation. -- Professional
liability insurance. -- Income tax withholdings. -- Social security
payments. The parties agree that the contractor shall not be considered
a VAMC employee for any purpose. 10. CONTRACT PERFORMANCE: a. The
contractor will be monitored through a variety of mechanisms, including
but not limited to: -- Clinic appointment lists showing patients seen.
-- sign-in/out log. -- Chart documentation of patient diagnosis,
treatment and or follow-up procedure. -- QA monitors designed to assess
the quality of care provided in the clinic. b. The Chief of Radiology
will be responsible for monitoring the quality of services provided
under this contract. 11. CLAUSES The provisions at FAR 52.212-1,
Instructions of Offeror -- Commercial Items (Mar 2000) applies to this
acquisition. There is not an addenda to these provisions. The clauses
at FAR 52.212-4, Contract Terms and Conditions -- Commercial Items
(May 1999) applies to this acquisition. There is an addenda to this
clause. Add the following clauses: FAR 52.252-2, Clauses Incorporated
by Reference (Feb 1998), This contract incorporates one or more clauses
by reference, with the same force and effect as if they were given in
