This is a combined solicitation for commercial items prepared in accordance with the information in FAR Subpart 12.6 as supplemented with the additional information included in this notice. This solicitation incorporates provisions and cluases that are in effect through Federal Acquisition Circular 46. This is a Request for Proposal for Eclipse TMR 2000 Holimium Laser System at the VA Medical Center, Miami, FL. The announcement constitutes the only solicitation being issued. Proposals are requested for RFP 546-20-00. Vendor will supply a mobile, self-contained, internally-cooled laser system intended for use in the operating room for laser tranmyocardial revascularization (TMR). System Specifications include, pulsed, solid-state holmium (Ho:YAG) laser, wavelength: 2.1 microns, mid-infrared, NHZ, energy per pulse: 4 Joules/pulse max., 20 watts power, 5 Hertz pulse rep rate, 250 microseconds pulse width, Fiber Optic system, air cooled, waterproof footpedal and other accessories, and warranty. The government intends to award a firm-fixed price contract based on price, installation, and maintenance services. The following clauses apply: 52.225-18, Buy American Act (41 U.S.C. 10); Provision 52.212-1, Instruction of offers-Commercial Items; 52.212-2, Evaluation-Commercial Items; 52.212-4, Contract Terms and Conditions-Commercial Items; and 52.212-5 apply to this acquisition.
