This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in the Federal Acquisition Regulations (FAR) Subpart 12.6, as supplemented with additional information included in this notice. (i) This CBD notice can be viewed and downloaded using the Agency Web Site. The Worldwide Web address is http://www.info.usaid.gov. Select Business and Procurement Opportunities from the home page, the "USAID Procurements". On the following screen, select "Download Available USAID Solicitations". (ii) The Solicitation Number for this Request For Quotations (RFQ) is OFDA-00-FW. This acquisition will be conducted using simplified acquisition procedures. (iii) The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC)90-17. (iv) The Standard Industrial Classification (SIC) code is 2326, with a small business size standard of 500 employees. This procurement is set aside for small businesses. (v) The U.S. Agency for International Development (USAID), Bureau for Humanitarian Response, Office of U.S. Foreign Disaster Assistance (OFDA), anticipates awarding a Purchase Order for uniforms intended for use by its members of Disaster Assistance Response Teams (DART). It is expected that the requirement will be for 100 of each of the six items. Vendors are required to submit one (1) sample of each item in each size (Small, Medium, Large, X-Large as applicable) with their quotation. The samples do not need to have the custom artwork. These samples will be returned only upon request. Successful vendor will be required to provide a prototype, with artwork of each type of shirt, cap, and vest, for OFDA approval. The USAID hand clasped emblem can be viewed at www.info.usaid.gov. The items are as follows: A. Hat, ball cap-style, with adjustable strap; navy blue color Ball caps shall have the three color (red, white, blue) USAID clasped hands logo embroidered on front of cap (2 3/8" high -- 1 7/8" wide). The back of ball cap (above adjustable strap) shall have the letters DART embroidered, " high, in white lettering. B. 1) Long and short sleeve shirt; collared with three button front pleat, navy blue color, with white silkscreen capital letters, and the three color (red, white, blue) USAID clasped hands logo shall be embroidered 2) Long and short sleeve shirt; collared with three button front pleat, light gray color, with dark blue silkscreen capital letters and the three color (red, white, blue) USAID clasped hands logo shall be in embroidered 3) Short and long sleeve T-shirt; navy blue color, with white silkscreen capital letters, and the three color (red, white, blue) USAID clasped hands logo shall be in silkscreen 4) Short and long sleeve T-shirt; light gray in color; with dark blue silkscreen capital letters and the three color (red, white, blue) USAID clasped hands logo shall be in silkscreen. All shirts shall be 100% cotton, pre-shrunk, medium quality and have the three color (red, white, blue) USAID clasped hands logo embroidered/silkscreen as appropriate, on left front over heart (2 3/8" high -- 1 7/8" wide). The letters DART, shall be embroidered/silkscreen as appropriate, on front 3/8" below clasped hand artwork, 1 " wide X " high. The shirts' backs shall have the word USAID in silkscreen lettering (centered 4" down from neck opening and 3 " high). The word HUMANITARIAN will be capital lettering above the logo not more than (11 " wide and 7/8" high), while the word RESPONSE will be screened below the logo in the same manner and approximate dimension. C. Work vest (similar to a photographer's vest); navy blue color The Vest shall be approximately 27" long with four front pockets (upper right side, upper left side, lower right side, and lower left side). Lower pockets shall include a separate side entry for hands. The back shall have a single pouch type zippered pocket. The vest shall be lined with mesh and shall have 4" adjustable waist straps on each side. The Vest shall have the three color USAID embroidered patch, 4" high by 3" wide, Velcro attached on the left-front pocket over heart. The vests' back shall have the USAID acronym in silkscreen lettering (centered but 4" down from neck opening, 11 " wide and 3 " high) in white with capital lettering. The word HUMANITARIAN will be screened in white above the logo not more than (11 " wide and 7/8" high), while the word RESPONSE will be screened in white below the logo in the same manner and approximate dimension. (vi) This notice is a request for competitive quotations. The Government will not be responsible for any cost incurred in responding to this notice. FAR provision 52.212-2 "Evaluation -Commercial Items"; the Government may award a single contract resulting from this solicitation to the responsible quoter whose quote conforming to the solicitation will most advantageous to the Government. The following evaluation criteria will be used to evaluate quotes: price and conformity to technical specifications.
