Q -- RADIOLOGIST SOL RFQ 124-0007 DUE 072800 POC Richard Houser, Contracting Officer 6085846120 The Federal Correctional Institution, Oxford, Wisconsin is soliciting to establish a contract with a Radiologist to provide specialty medical services in the area of radiological medicine to inmates incarcerated at the Federal Correctional Institution and Federal Prison Camp. The board certified/eligible radiologist will serve as Director of Radiology Services and shall provide evaluations and interpretations of roentgenograms, and special diagnostic roetgenographic procedures to include arthrograms, barium enemas, G.I. Series, venograms, cholecystograms, and ultrasounds approximately 72 each. The contractor will provide specialized radiological tests and procedures on patients on referral from the institution medical staff. The contradtor will provide written and/or dictated reports on all patients referred for evaluation and will include recommendations for further diagnostic testing and/or treatment, when indicated. Contractor will provide one, two hour session, per week as needed for an estimated 36 sessions for the estimated contract period of October 1, 2000 through September 30, 2001. Offerors current State of Wisconsin licensure and/or certification to be considered for award. Offerors must have liability insurance issued by a responsible carrier in order to be eligible for award. A firm fixed-price contract will be awarded on or about September 01, 2000 to the lowest qualified bidder. This acquisitioin is 100% set-aside for small business.
