of Veterans Affairs, VA Gulf Coast Veterans Healthcare System a
Digital Retinal Imaging System to be used in documenting diseases of
the eye. This is a combined synopsis/solicitation for a commercial
item, prepared in accordance with the format in subpart 12.6, as
supplemented with additional information included in this notice. This
notice constitutes the only solicitation; quotations are being
requested and a written solicitation will not be issued. Solicitation
number/ Request for Quotes (RFQ) 520-32-00 is assigned and shall be
referenced on any quotation. This is a Request for Quotation for which
it and all incorporated provisions and clauses are those in effect
through Federal Acquisition Circular 97-77. This procurement is 100%
Set-Aside for small business. The Standard Industrial Classification
Code (SIC) is 5047. The small business size standard for this
procurement is 500. The contractor shall provide a digital retinal
imaging system that is a complete turn key system that is portable,
high resolution FA/ICG (color & black & white), DICOM compliant must be
compatible with the Government owned VISTA Computer System, which
includes the following minimum requirements: Windows NT based computer
(tower system) 650 MHZ, with 21" high resolution, color monitor, 2 ea.
4.1 Gig HD 128 MB RAM, 16 Mbytes controller card, 3-D graphics adapter,
high speed modem (56K Baud), communication and utility software, CD
archiving (Write 8X/Read24X), CD reader 32X, battery back up, power
regulator, 1280x 1024 image card, color imaging board, carbon copy
software, anti virus, power supply, dual TV relay lens (B&W), B&W
camera for FA/ICG and ICG alignment camera, color 3 chip video camera,
digital color 800 X 600, 2 sets of cables, FA/ICG Fundus Camera,
automatic triple mount CCD camera interface that will allow automatic
switching between cameras, extra set of cables (spare), 2 each camera
backs, FA filter set, ICG filter set, adjustable instrument table,
height adjustment of 550mm-770mm, dimensions, 490 (w) x 490 (D).
Contractor to provide initial set up and two (2) days training. The
following FAR provisions apply and are incorporated by reference: FAR

Advertising. Offerors shall provide a complete set of provisions with
quotation found at FAR 52.212-3, Offeror Representation and
Certifications-Commercial Items, FAR 52.212-5 along with the following
additional FAR clauses sited in clause FAR 52.212-5: FAR 52.222-26

brand name, model, and warranty for each unit being offered. NOTE:
Descriptive Literature: Offerors are cautioned that the responsiveness
of their quotation depends on the completeness and sufficiency of
descriptive material submitted with the quotation. Bidders must
highlight descriptive literature to show the item meets the minimum
requirements. The Offeror shall submit certification to this VA Medical
Center that the items being offered are Year 2000 Compliant. Price must
be FOB destination, Biloxi, MS. Award will be based on price. Proposals
shall be submitted on an "all or none basis." Partial proposals shall
be rejected. All offers are due by Close of Business July 27, 2000
(4:30PM CST). Offers in original and one (1) copy along with a
completed copy of FAR 52.212-3 and VAAR 852.219-70, should be sent to:
