combined synopsis/solicitation for commercial services, Scarce Medical
Specialist Resources through Locum Tenems for a UROLOGIST, prepared in
accordance with the format in FAR Subpart 12.6 and 13.5 as supplemented
with the additional information included in this notice. Procurement of
medical personnel is authorized by 38USC 8153. This announcement
constitutes the only solicitation issued; quotes are being requested
and a written solicitation will not be issued. This acquisition is
unrestricted and all responsible sources may submit a quotation for
consideration. Quotations shall be considered only if clear and
convincing evidence is provided that the offeror can meet the
requirements of the Government in all areas listed. Solicitation number
is 502-21-00 and is Request for Quotation (RFQ). Award will be made
using simplified acquisition procedures. This solicitation documents
and incorporates provisions and clauses in effect through Federal
Acquisition Circular 97-18 and VAAR #20. The standard industrial
classification code is 7361 with $5 annual sales limitation for small
business. Requirements are One (1) Medical Doctor Urologist for an
estimated 400 hours. Services required for the period July 17, 2000 (or
as soon as possible) through September 30, 2000, with an option to
extend an additional three (3) months. 400 hours @ $ ___________. Work
schedule is Monday through Friday, 8:00 AM to 4:30 PM and excludes
established holidays and any other declared national holiday. One
physician for the duration of the contract term is preferred for
continuity of care. No overtime, callback or holiday coverage included.
The Department of Veterans Affairs Medical Center prefers Board
Certified or Board Eligible with less than four years post residency.
Urologist is needed pending completion of recruitment for full time
staff physician. The Urologist shall be responsible for comprehensive
routine and specialized Genitourinary care, both inpatient and
outpatient, including evaluation and assessment, diagnosis, management,
treatment plans, documentation, telephone consultations, and
prescribing VA formulary drugs. It may also include limited surgical
procedures such as cystoscopy and biopsy. The Urologist shall be the
primary urologist rendering care and shall possess sufficient
knowledge, skills and experience to act independently with minimal
supervision. Workload requires clinic-setting services for
approximately 25-30 patients daily with varying degrees of conditions
and diagnoses, at the Department of Veterans Affairs Medical Center, PO
Box 69004, 2495 Shreveport Highway, Alexandria LA. Knowledge and
capability to document electronically in accordance with VA standards
is essential. All services shall be provided in accordance with all
rules and regulations as set forth in VHA Directives, Medical Staff
Bylaws, Medical Center Memorandums, JCAHO standards, and normal quality
standards of the Urology profession. Services performed shall be under
the direction of the Director of Specialty Care Health HCL and the
Chief of Staff. Teamwork is essential. All rules, regulations and laws
applicable to privacy of patient, documentation and records apply.
Preferred service dates are 7/17/00 thru 9/30/00 with an option to
extend for an additional ninety days and an option to terminate sooner
if VA recruitment efforts are successful. Payment shall be weekly in
arrears using Government purchase card. Logs of work hours (certified
sign-in/sign-out logs maintained by Specialty Care) will be faxed
weekly to Contractor. Contractor shall fax corresponding invoice to
318-483-5063 and obtain credit card information. Payment shall not
exceed 40 hours per week. The hourly rate is an all-inclusive rate and
no additional consideration will be made. Qualifications: Urologist
assigned by the Contractor to perform requested services shall be
licensed in a state, territory, or commonwealth of the United States or
District of Columbia with current full, unchallenged and unrestricted
license; VA prefers board certified or board eligible with less than
four years post residency. The contract physician shall speak,
understand, read and write the English language fluently; be computer
literate with knowledge of DHCP/VISTA electronic medical records
documentation access and input. He/she shall be a US citizen and must
possess required elements to be credentialed and privileged in
accordance with VA regulations. Physician credentials/qualifications
are subject to review and approval of the Medical Center Chief of Staff
and Medical Center Director. The Credentialing Officer for VAMC
Alexandria shall be provided all requested documentation to credential
and privilege the physician prior to acceptance at this facility. OIG
List of Excluded Individuals & Entities applies. Hours are Monday
through Friday, excluding all federal holidays, 8:00 AM to 4:30 PM. No
overtime, weekends, or callbacks authorized. One physician is
preferred for the entire contract period. The ten holidays observed by
the government include New year's Day, Martin Luther Day, President's
Day, Memorial Day, Independence Day, Labor Day, Columbus Day, Veterans
Day, Thanksgiving, Christmas and any other day proclaimed by the
President of the United States to be a national holiday. Personnel
Policy: The contractor shall be responsible for protecting contracted
personnel assigned to this facility. The contractor shall provide
worker's compensation, professional liability insurance in the amount
of $1,000,000 per occurrence, health examinations, income tax
withholding and social security payments. Sick leave, vacation, or
other benefits are not the responsibility of the Government. At no time
shall the contracted physician be considered a VA employee for any
purpose. Contractor Performance Monitoring: Monitoring of hours worked
will be accomplished with sign-in/sign-out sheets maintained by the
secretary of Specialty Care Healthcare Line (SCHCL) and verified by the
Director of SCHCL documenting arrivals and departures. The Director of
SCHCL shall be designated Contracting Officer's Technical
Representative and shall monitor performance for contract compliance,
knowledge, skills and behavior and immediately notify the Contracting
Officer of any noncompliance or unacceptable behavior. Substitution of
physicians is not allowed unless specifically authorized by the
Contracting Officer, Director of SCHCL, and the Chief of Staff. Any
substitute must comply with all requirements and obtain credentialing
and privileging. The Contractor shall submit CV, at least three
references with names and current phone numbers, certifications,
affiliations, previous employers and documentation sufficient to
complete credentialing and privileging process (education, board
certification, relevant training and experience, references, pending or
settled challenges to license, status of any claims resolved or
pending). The Government may award from documentation provided with
quote without further discussion. SUBMIT ALL BACKUP DOCUMENTATION FOR
CONSIDERATION WITH OFFER. Award will be made to theofferor whose quote
conforms to the requirements of the solicitation; best meets the needs
of the Government and offers the lowest price. Best value to the
Government and veteran shall determine award considering past
performance, technical/qualifications and cost. Each factor shall be
considered equal in evaluation and rating. It is preferred to issue a
single firm-fixed-cost-contract.Provisions at 52.212-1 Instructions to
Offerors-Commercial Items (NOV 99) without addendum applies to this
acquisition. Clauses at 52.212-4, Contract Terms and Conditions (MAY
