Programs is conducting a market survey to determine the existence of
businesses capable of providing hospital nutrition/meal services for
the HD patients residing in government-leased 30-bed long term health
care facility located at Summit Hospital, Baton Rouge LA. THIS IS A
MARKET SURVEY ONLY. THERE IS NO SOLICITATION AVAILABLE AT THIS TIME.
REQUESTS FOR A SOLICITATION WILL NOT RECEIVE A RESPONSE. The potential
services to be performed include supplying labor, materials, supplies
and transportation for complete hospital meal and nutrition service,
7 days week, 365 days a year (including, but not limited to, regular
diets, soft diets, pureed diets, High CHO, High Protein diets, diabetic
diet, 2 GM sodium diets, bedtime snacks, nourishment, yogurt, juice,
and meal supplements/substitutes such as Ensure, Promod, Jevity and
others as specified by physicians). Food must be prepared and served in
accordance with JCAHO requirements. THERE ARE NO FOOD PREPARATION,
SERVING TRAY PREPARATION, SERVING, STORAGE/ REFRIGERATION, CLEAN UP,
FOOD WARMING, REFRIGERATION OR DISPOSAL FACILITIES WITHIN THE
GOVERNMENT LEASED SPACE. Food to be prepared in contractor spaces and
delivered to the unit on serving carts with food trays ready to be
served to patients for breakfast, lunch, dinner, snacks and special
needs at specified times for each meal. The geographical zone of
consideration for the contractor's food preparation site is within 5
miles of Summit Hospital, Baton Rouge LA. Interested and qualified
organizations within the geographical zone of consideration, with a
demonstrated past performance providing this particular type of food
service are requested to provide a statement of current capability and
past performance information. Sources must demonstrate management and
technical experience on similar acquisitions, specifically addressing
the requirements identified in this synopsis, and demonstrate the
ability to obtain the resources necessary to successfully compete for
this requirement. Interested parties are requested to provide the
following information: (1) Company name, point of contact, physical
address, e-mail address, telephone and facsimile numbers, and type and
business size; (2) A list of your most recent and relevant contracts
(government and/or commercial), not to exceed five, including names,
points of contact, phone numbers, value and type of contract, period of
performance and brief description; (3) Information telling about your
company and why you think you could perform the described services; (4)
Information describing works, tasks and percentages to be performed by
you and subcontractors. Interested business sources should provide the
experience they have in management of this type services and experience
in performing the requirements. A letter of interest should also
include and address the following: (1) Name and address of the firm (2)
Size of business including annual revenue for this type service and
annual revenue for entire business (3) Total number of employees (4)
Year firm established (5) Name of tow principals to contact, including
title and telephone number (6) previous experience. Please include
pertinent information for on-going contracts or contracts completed
within the last three years. Pertinent information is (a) Name of
contracting activity (b) Contract type (FFP, cost, IDIQ or combination
(d) Period of performance and total contract value (e) Summary of
contract work (f) Contracting Officer name and current telephone
number. This synopsis is for the sole purpose of identifying potential
sources and does not commit the Government to issuing a solicitation
or making an award. Interest parties should provide this information by
July 18, 2000. All information submitted is at the respondent's own
expense. The Government will not reimburse respondents for any cost
associated with submission of capability statements in connection with
