commercial services prepared in accordance with the format in Subpart
12.6 as supplemented with additional information included in this
notice. This announcement constitutes the only solicitation; quotes are
being requested and a written solicitation will not be issued. The
solicitation is being issued as Request for Quote #565-27-00. The
solicitation document and incorporated provisions and clauses are those
in effect through Federal Acquisition Circular 97-15. The SIC code is
7363 and the small business size standard is $5.0 million. The VA
Medical Center, Fayetteville, N.C. has an urgent need for services of
a board certified psychiatrist and intends to solicit quotes and award
a firm fixed price contract under the authority of Title 38 USC
Section 8153, pending availability of funds. Contract period will be
established from date of award for the period of 90 days with option to
extend. Estimated start date mid-August, contingent upon credentialing
of candidate. WORK STATEMENT: Mental Health Service Line --
Psychiatrist 1. Provide the services of a board certified or board
eligible Psychiatrist to perform all duties and responsibilities of a
staff psychiatrist. Also, this physician should have experience in the
area of chemical addiction treatment. Physician must be either ACLS or
BCLS certified. 2. Duties will include: a. Treatment and professional
direction of outpatient/inpatient care; and outpatient/inpatient care
consultations. b. Provide professional direction and participate in the
treatment team to evaluate; develop and implement treatment plans. c.
Develop and implement follow-up plans. d. Participate in organization
meetings and duties of the VA Medical Center as required. 3. Clinical
competence will include: -Psychiatric Examinations -Diagnosis
Formulation -Therapeutic Plan -Patient Management -Consultations
-Record Keeping Psychiatrist must have current practice experience with
acute outpatient and inpatient treatment with working knowledge of
current pharmacology. In addition, approximately 20 hours per week will
involve professional direction and involvement with the Chemical
Addiction Rehabilitation Program treatment team. Physician should have
had recent direct experience in chemical addiction treatment programs.
4. The psychiatrist may provide on-call duties at night an on weekends
in rotation with other psychiatric staff and answer inpatient
consultations only if authorized by the Contracting Officer. Other
duties, as assigned by mutual agreement between the locum tenens and
the Acting Chief, Psychiatry Service. 5. Contractor will act in
compliance with all Rules and Regulations of the Fayetteville VA
Medical Center. The contract physician will be required to enter all
reports and incorporation into the patient's record and will satisfy
VHA and medical center policies. 6. The psychiatrist must meet the
requirements for appointment to the medical staff, VAMC, Fayetteville,
N.C. as outlined in the practice in one of the states or territories
of the United States or District of Columbia. The qualifications of
such personnel shall also be subject to the review by the Chief, of
Staff and approval by the VA facility Director. Personnel must meet the
credentialing/privileging requirements of the VA Medical Center,
Fayetteville, N.C. COST PROPOSAL: Provide all inclusive rate covering
all fees, professional liability insurance, any housing and
transportation costs involved including hourly rate; overtime rate; on
call availability and rate. Personnel assigned by the contractor to
perform the services covered by this contract shall be licensed in a
State, Territory, or Commonwealth of the United States or the District
of Columbia. Personnel performing these services shall be a board
certified/eligible and ACLS or BCLS certified and must meet the
credentialing privileging requirements of the VA Medical Center,
Fayetteville, N.C. Services to be performed by the contractor will be
under the direction of the Chief of Staff or his/her designee at the VA
Medical Center, Fayetteville, N.C. The following information is
provided as general information. The contractor under the terms of the
contract shall furnish the work hours as mutually agreed upon in the
work statement. Work hours: Monday through Friday -- 8:00 a.m. -- 4:30
p.m. National Holiday: The ten holidays observed by the Federal
Government, i.e. New Year's Day, Martin Luther King's Birthday,
Washington's Birthday, Memorial Day, Independence Day, Labor Day,
Columbus Day, Veterans Day, Thanksgiving, and Christmas Day and any
other day specifically declared by the President of the United States
to be a national holiday. Nonworking hours (normal on-call hours)
Monday through Friday, 4:30 p.m. to 8:00 a.m., and all Saturdays and
Sundays. Emergency On-Call Hours: 12:00 Midnight Sunday through 12:00
Midnight the following Sunday. Work performed without prior approval of
the Contracting Officer is not reimbursable. The contractor shall be
responsible for protecting the personnel furnishing services under this
contract. To carry out this responsibility, the contractor shall
provide the following for their personnel: worker's compensation,
professional liability insurance, health examinations, and income tax
withholding and social security payments. The parties agree that such
personnel shall not be considered a VA employee for any purpose and
shall be considered employees of the contractor. Payment for any leave,
including sick leave, holiday or vacation time is the responsibility of
the contractor. The following provisions apply to this solicitation:

52.212-2 Evaluation-Commercial Items; Offerors responding to this
announcement shall submit a cost proposal and technical proposal to
include the following evaluation factors company experience, personnel
qualifications to include evidence of current licensure and
credentials/privileges and current CV of candidate, Indemnification and
Medical Liability Insurance certificate, past performance of contracts
of similar type, scope size and complexity that are ongoing or
completed within past three years, references with points of contacts
for contracts identified. Technical and past performance, when
combined, are significantly more important than cost or price. Award
will be made to the offeror who offer conforming to the solicitation
will be most advantageous to the Government, price and other factors
considered. The government intends to evaluateoffers and award without
discussions. However, the government reserves the right to conduct
discussions later if determined necessary by the Contracting officer.
